________________(以下稱甲方)________________(以下稱乙方)甲方為了滿足廣大人民群眾的需要,更好的為人民的健康提供服務(wù),經(jīng)雙方協(xié)商,本著平等,互惠,互利的原則,現(xiàn)承包醫(yī)院口腔科給乙方第一條: 經(jīng)營(yíng)期限:自________年____月____日起至________年____月____日止，將__________醫(yī)院口腔科租給_________乙方負(fù)責(zé)修理并承擔(dān)費(fèi)用。第二條

自本合同生效之日起，乙方須按季度預(yù)付租金________元。乙方接到甲方的收租通知單后，在三十天內(nèi)一次付清。逾期須按日支付千分之________的滯納金，以補(bǔ)償甲方所受的損失。第三條
