中國智利原產(chǎn)地證書

辦理中國-智利自由貿(mào)易產(chǎn)地證MOB:***QQ:858651817 《中國—智利自由貿(mào)易區(qū)》原產(chǎn)地證書 FORM F 自2006年10月1日起，各地出入境檢驗檢疫機構(gòu)開始簽發(fā)《中國-智利自由貿(mào)易區(qū)優(yōu)惠原產(chǎn)地證明書》（FORM F），該日起對原產(chǎn)于我國的5891個6位稅目產(chǎn)品關(guān)稅降為零。中國—智利自由貿(mào)易區(qū)原產(chǎn)地證書采用專用格式.證書號:證書標題欄（右上角），填上檢驗檢疫機構(gòu)編定的證書號。（證書號編定規(guī)則例如： ZJFA33121/060001）

第一欄：注明出口人合法的全稱、地址（包括國家）。

第二欄：注明生產(chǎn)商合法的全稱、地址（包括國家）。如果證書包含一個以上生產(chǎn)商的商品，應該列出其他生產(chǎn)商的名稱、地址（包括國家）。如果出口人或生產(chǎn)商希望對該信息予以保密，可以填寫“應要求提供給主管政府機構(gòu)（Available to competent governmental authority upon request）”。如果生產(chǎn)商和出口人相同，應填寫“同上（SAME）”。如果不知道生產(chǎn)商，可填寫“不知道（UNKNOWN）”。

第三欄：注明收貨人合法的全稱、地址（包括國家）。

第四欄：注明運輸方式和路線，并詳細說明離港日期、運輸工具號、裝貨港和卸貨港。

第五欄：由進口方的海關(guān)當局在該欄簡要說明根據(jù)協(xié)定是否給予優(yōu)惠待遇。第六欄：如有要求，該欄可注明客戶訂單編號、信用證編號等。如果發(fā)票由非成員方經(jīng)營者出具，應在此欄填寫原產(chǎn)國生產(chǎn)商的名稱、地址及國家。此欄不能留空，無填寫內(nèi)容填打“***”

第七欄：注明項目編號，但不得超過20個。

當項目超過20個時，應使用新一頁FORM F證書作續(xù)頁，續(xù)頁從第七欄起開始添制

第八欄：注明包裝上的運輸嘜頭及編號。

此欄不能留空，無嘜頭內(nèi)容填打“N/M”

第九欄：注明包裝數(shù)量及種類，并列明每種貨物的詳細名稱，以便查驗的海關(guān)官員可以識別，并使其能與發(fā)票上的貨名及HS編碼上的貨名相對應。如果貨物無包裝，應注明“散裝（IN BULK）”。貨物描述結(jié)束后，應在后面添加“***”或“＼”（截止符）。

第十欄：對應第九欄中的貨物名稱注明相應的協(xié)調(diào)制度編碼，以六位編碼為準。

第十一欄：若貨物符合原產(chǎn)地規(guī)則，出口人必須按照下表中規(guī)定的格式，在本證書第十一欄中注明其申報貨物享受優(yōu)惠待遇所依據(jù)的原產(chǎn)地標準。出口人申報其貨物享受優(yōu)惠待遇所依據(jù)的原產(chǎn)地標準-------填入第11欄

（1）完全原產(chǎn)---------“P”

（2）含進口成份，區(qū)域價值≥40%---------------------“RVC”

（3）產(chǎn)品特定原產(chǎn)地標準-----------------------------“PSR”

第十二欄：該欄應注明毛重的公斤數(shù)，其他按慣例能準確表明數(shù)量的計量單位，如體積、件數(shù)等也可用于該欄。

第十三欄：應注明發(fā)票號、發(fā)票日期及發(fā)票價值，發(fā)票價值填寫FOB價。發(fā)票價值列項應與第七欄、第九欄、第十欄、第十一欄和第十二欄對應第十四欄：該欄必須由出口人填寫，簽名、簽署日期和加蓋印章。

第十五欄：該欄必須由簽證機構(gòu)經(jīng)授權(quán)的簽證人員簽名、簽署日期并加蓋簽證印章，并應填寫簽證機構(gòu)的電話號碼、傳真及地址。本公司專業(yè)快速辦理FORM F原產(chǎn)地證書以及以下業(yè)務：

中國-智利自貿(mào)區(qū)協(xié)定項下的原產(chǎn)地證書應在自出口方簽發(fā)之日起一年內(nèi)有效。

6背面說明

第一欄：應填寫出口商詳細的依法登記的名稱、地址（包括國家）。

第二欄：在已知的情況下填寫生產(chǎn)商詳細的依法登記的名稱、地址（包括國家）。如果證書包含一

個以上生產(chǎn)商的商品，應該列出其他生產(chǎn)商的詳細名稱、地址（包括國家）。如果出口商或生產(chǎn)商希望對信息予以保密，可以填寫“應要求提供給簽證機構(gòu)（Available to competent governmental authorityupon request）”。如果生產(chǎn)商和出口商相同，應填寫“相同（SAME）”。如果不知道生產(chǎn)商，可填寫“不知道（UNKNOWN）”。

第三欄：應填寫收貨人詳細的依法登記的名稱、地址（包括國家）。

第四欄：應據(jù)所知填寫運輸方式及路線、詳細說明離港日期、運輸交通工具的編號、裝貨口岸和

到貨口岸。

第五欄：不論是否給予優(yōu)惠待遇，進口方海關(guān)必須在相應欄目標注(√)。

第六欄：如有要求可以填寫顧客順序號、信用證號等。如果發(fā)票是由非締約方開出的，應在此欄

標注原產(chǎn)國生產(chǎn)商的名稱、地址和國家。

第七欄：應填寫項目號，但不得超過20項。第八欄：應填寫嘜頭及包裝號。

第九欄：應詳細列明包裝數(shù)量及種類。對每種貨物提供詳細的貨物描述，以便于查驗的海關(guān)關(guān)員

可以識別。貨物描述應與發(fā)票描述及貨物的協(xié)調(diào)制度編碼相符。如果是散裝貨，應注明“散裝”。當貨物描述結(jié)束時，加上“***”（三顆星）或“＼”（結(jié)束斜線符號）。

第十欄：應對應第九欄中的每種貨物填寫協(xié)調(diào)制度編碼，以六位編碼為準。

第十一欄：若貨物符合原產(chǎn)地規(guī)則，出口商必須按照下列表格中規(guī)定的格式，在本證書第十一欄

量。

第十三欄：應填寫發(fā)票號、發(fā)票日期及發(fā)票價。

第十四欄：本欄必須由出口商填寫、簽名并填寫日期。且應該填寫簽名的地點及日期。

第十五欄：本欄必須由簽證機構(gòu)的授權(quán)人員填制、簽名、填寫簽證日期并蓋章。并應提供簽證機構(gòu)的電話號碼，傳真及地址。

2．method of reimbursement 索償辦法

（1）in reimbursement, we shall authorize your Beijing Bank of China Head Office to debit our Head Office RMB Yuan account with them, upon receipt of relative documents 償付辦法，我行收到有關(guān)單據(jù)后，將授權(quán)你北京總行借記我總行在該行開立的人民幣帳戶

（2）in reimbursement draw your own sight drafts in sterling on…Bank and forward them to our London Office, accompanied by your certificate that all terms of this letter of credit have been complied with 償付辦法，由你行開出英鎊即期匯票向……銀行支取。在寄送匯票給我倫敦辦事處時，應隨附你行的證明，聲明本證的全部條款已經(jīng)履行

（3）available by your draft at sight payable by us in London on the basis to sight draft on New York 憑你行開具之即期匯票向我行在倫敦的機構(gòu)索回票款，票款在紐約即期兌付

（4）in reimbursement, please claim from our RMB￥ account held with your banking department Bank of China Head Office Beijing with the amount of your negotiation 償付辦法，請在北京總行我人民幣帳戶中索回你行議付之款項

原產(chǎn)地證明書（CERTIFICATE OF ORIGIN）是出口商應進口商要求而提供的、由公證機構(gòu)或政府或出口商出具的證明貨物原產(chǎn)地或制造地的一種證明文件。原產(chǎn)地證書是貿(mào)易關(guān)系人交接貨物、結(jié)算貨款、索賠理賠、進口國通關(guān)驗收、征收關(guān)稅的有效憑證，它還是出口國享受配額待遇、進口國對不同出口國實行不同貿(mào)易政策的憑證。2類型編輯

根據(jù)簽發(fā)者不同，原產(chǎn)地證書一般可分為以下三類： ①商檢機構(gòu)出具的原產(chǎn)地證書，如：中華人民共和國檢驗檢疫局（CIQ）出具的普惠制產(chǎn)地證格式A（GSP FORM A）；一般原產(chǎn)地證書（CERTIFICATE OF ORIGIN）。

②商會出具的產(chǎn)地證書，如：中國國際貿(mào)易促進委員會（CCPIT）出具的一般原產(chǎn)地證書，簡稱貿(mào)促會產(chǎn)地證書（CCPIT CEERTIFICATE OF ORIGIN）；

③制造商或出口商出具的產(chǎn)地證書；

在國際貿(mào)易實務中，應該提供哪種產(chǎn)地證明書，主要依據(jù)合同或信用證的要求。一般對于實行普惠制國家出口貨物，都要求出具普惠制產(chǎn)地證明書。如果信用證并未明確規(guī)定產(chǎn)地證書的出具者，那么銀行應該接受任何一種產(chǎn)地證明書?，F(xiàn)在我國多數(shù)出口商習慣于使用貿(mào)促會出具的證書。3申請編輯

根據(jù)我國的規(guī)定，企業(yè)最遲于貨物報關(guān)出運前三天向簽證機構(gòu)申請辦理原產(chǎn)地證書，并嚴格按照簽證機構(gòu)的要求，真實、完整、正確地填寫以下材料：

①《中華人民共和國出口貨物原產(chǎn)地證書/加工裝配證明書申請書》；

②《中華人民共和國出口貨物原產(chǎn)地證明書》一式四份； ③出口貨物商業(yè)發(fā)票；

④簽證機構(gòu)認為必要的其他證明文件。

⑴ 進出口商的名稱及地址； ⑵運輸方式及航線； ⑶商品嘜頭和編號； ⑷商品名稱、數(shù)量和重量； ⑸證明文字等。

填寫出口公司的詳細地址、名稱和國家（地區(qū)）名。若經(jīng)其他國家或地區(qū)，需填寫轉(zhuǎn)口商名稱時，可在出口商后面填英文VIA，然后再填寫轉(zhuǎn)口商名稱、地址和國家。

⑶收貨方（CONSIGNEE）

填寫最終收貨人名稱、地址和國家（地區(qū)）名。通常是外貿(mào)合同中的買方或信用證上規(guī)定的提單通知人。如信用證規(guī)定所有單證收貨人一欄留空，在這種情況下，此欄應加注“TO WHOM IT MAY CONCERN”或“TO ORDER”，但此欄不得留空。若需填寫轉(zhuǎn)口商名稱時，可在收貨人后面加填英文VIA，然后再寫轉(zhuǎn)口商名稱、地址、國家。

⑷運輸方式和路線（MEANS OF TRANSPORT AND ROUTE）填寫裝運港和目的港、運輸方式。若經(jīng)轉(zhuǎn)運，還應注明轉(zhuǎn)運地。例如：通過海運，由上海港經(jīng)香港轉(zhuǎn)運至漢堡港，應填為： FROM SHANGHAI TO HAMBURG BY VESSEL VIA HONGKONG。

⑸目的地國家（地區(qū)）（COUNTRY/REGION OF DESTINATION）

填寫目的地國家（地區(qū)）。一般應與最終收貨人或最終目的港（地）國別相一致，不能填寫中間商國家名稱。

⑹簽證機構(gòu)用欄（FOR CERTIFYING AUTHORITY USE ONLY）由簽證機構(gòu)在簽發(fā)后發(fā)證、補發(fā)證書或加注其他聲明時使用。證書申領(lǐng)單位應將此欄留空。一般情況下，該欄不填。

⑺運輸標志（MARKS AND NUMBERS）填寫嘜頭。應按信用證、合同及發(fā)票上所列嘜頭填寫完整圖案、文字標記及包裝號碼，不可簡單填寫“按照發(fā)票”（AS PER INVOICE NUMBER）或者“按照提單”（AS PEER B/L NUMBER）。貨物如無嘜頭，應填寫“無嘜頭”（NO MARK）字樣。此欄不得空留，如嘜頭多，本欄目填寫不夠，可填寫在第7、8、9欄內(nèi)的空白處，如還是不夠，可用附頁填寫。

⑻商品描述、包裝數(shù)量及種類（NUMBER AND KIND OF PACKAGES；DESCRIPTION OF GOODS）

填寫商品描述及包裝數(shù)量。商品名稱要填寫具體名稱，不得用概括性表述，例如服裝、食品（GARMENT、FOOD）等。包裝數(shù)量及種類要按具體單位填寫，應與信用證及其他單據(jù)嚴格一致。包裝數(shù)量應在阿拉伯數(shù)字后加注英文表述。如貨物為散裝，在商品名稱后加注“散裝”（IN BULK）字樣。有時信用證要求在所有單據(jù)上加注合同號碼、信用證號等，可加注在此欄內(nèi)。本欄的末行要打上表示結(jié)束的符號（**************），以防加添內(nèi)容。

⑼商品編碼（HS CODE）

此欄要求填寫HS編碼，應與報關(guān)單一致。若同一證書包含有幾種商品，則應將相應的稅目號全部填寫。此欄不得空留。

⑽數(shù)量（QUANTITY）

此欄要求填寫出口貨物的數(shù)量及商品的計量單位。如果只有毛重時，則需填“G.W.”。

⑾發(fā)票號碼及日期（NUMBER AND DATE OF INVOICE）填寫商業(yè)發(fā)票號碼及日期。此欄不得空留，為避免對月份、日期的誤解，月份一律用英文表述，如2004年3月15日，則為： MARCH.15，2004。

⑿出口方聲明（DECLARATION BY THE EXPORTER）填寫出口人的名稱、申報地點及日期，由已在簽證機構(gòu)注冊的人員簽名并加蓋有中英文的印章。

⒀由簽證機構(gòu)簽字、蓋章（CERTIFICATION）

填寫簽證地點、日期。簽證機構(gòu)簽證人經(jīng)審核后在此欄（正本）簽名，并蓋簽證印章。6更改重發(fā)編輯

對簽證機構(gòu)已簽發(fā)的原產(chǎn)地證書，當申請單位需要更改其內(nèi)容時，申請單位應書面申明理由，提交已更改的產(chǎn)地證，并退回原證書正本。

對簽證機構(gòu)已簽發(fā)的原產(chǎn)地證明書遺失或損毀，申請單位應書面說明遺失或損毀的原因，提交重新填制的產(chǎn)地證書副本或復印件
