關于申辦自學考試畢業(yè)證、學位證的通知

一、畢業(yè)證申請時間2011年12月12日～12月16日。

二、畢業(yè)證申請地點陽明校區(qū)實驗樓五樓自考辦。（0791-87119888）

三、申報條件

1、完成全部課程考試且成績合格。

2、畢業(yè)論文和實踐考核成績合格。

四、畢業(yè)申請所需材料

1、第二代身份證原件及復印件一份，復印件上的姓名及身份證號必須清晰一致。

2、準考證原件。、第一學歷本科畢業(yè)證原件及復印件。

4、更改姓名、身份證號的考生，須提供公安機關出具的戶籍證明。

5、考生需準備1張以準考證號命名的電子照片，標準小2寸，藍底，免冠近照。

五、畢業(yè)申請流程

1、領取檔案袋填寫，并將材料統(tǒng)一放入檔案袋內。

2、考生在申請點現(xiàn)場等待工作人員審核，并確認自己網(wǎng)上畢業(yè)信息的正確，然后在“江西省2011年下半年自學考試畢業(yè)信息登記確認表”內填寫個人信息并確認簽字。信息有差錯的考生當場提出，由工作人員核查相關證明后，再進行修改和處理，信息修正后考生再次確認簽字。

六、注意事項

畢業(yè)申請原則上必須本人到現(xiàn)場辦理，若因工作地域原因，實在無法親自到場辦理，需寫委托書交給在昌同學或朋友代辦（委托書必須寫明無法到場原因，委托者的姓名與身份證號，并要求委托者帶好本人身份證前往陽明校區(qū)，最后一句寫明倘若因委托者核對被委托者畢業(yè)信息失誤造成的一切后果由被委托者個人承擔，將一律不予修改）。

七、畢業(yè)證發(fā)放時間將在QQ群及上公布

八、學位證申請時間：必須在次年的2-3月完成。按省自考辦規(guī)定，逾期將無法辦理。

九、學位證申請條件：

1、學位英語成績中作文≥13，且總成績合格（09年以前參考的作文≥6）

2、學位課程成績合格

十、申請學士學位所需材料：

1、自學第二學歷畢業(yè)證復印件一份

2、身份證復印件一份

3、三張1寸、一張2寸同底藍色底版照片，一份同底電子照片（照片到.cn下載）

4、認真填寫三張學位申請表（共享群已經(jīng)上傳）

十一、學位英語及學位課程考試必須在自考畢業(yè)證申請之前通過，畢業(yè)申請后不再允許報考。學位英語及學位課程每年考兩次。報考時間分別是上半年2－3月，考試時間為4月；下半年9月，考試時間為11月。建議考生提前參加學位英語考試，考試成績在畢業(yè)前均有效。

第三篇：畢業(yè)證和學位證區(qū)別
