“領(lǐng)導(dǎo)簽名”欄，企業(yè)法人代表或其委托領(lǐng)導(dǎo)須親筆簽名（企業(yè)經(jīng)辦人不可擅自代簽，也不可以蓋領(lǐng)導(dǎo)簽字章），后蓋公章。
“聯(lián)系人”要填寫企業(yè)經(jīng)辦人自己的聯(lián)系方法（為不耽擱您的寶貴時(shí)間，最好寫上手機(jī)號），而且要表中所寫的“聯(lián)系人”本人去金華取簽證通知，不可委托快遞公司取。
護(hù)照的復(fù)印件必須包含如下四項(xiàng)內(nèi)容：
