實(shí)施方案

根據(jù)湘教通湘教發(fā)〔2015〕4號(hào)關(guān)于印發(fā)《湖南省普通高等教育“專升本”工作實(shí)施辦法》的通知的文件精神，為做好我校2015年“專升本”工作，特制定“專升本”工作實(shí)施方案。

一、辦事機(jī)構(gòu)

為加強(qiáng)“專升本”工作的領(lǐng)導(dǎo)，使“專升本”工作嚴(yán)格按照省教育廳提出的推薦條件和選拔程序?qū)嵭腥瘫O(jiān)督，確?！皩Ｉ尽惫ぷ鲊?yán)格按照公開、公平、公正的原則進(jìn)行，學(xué)?！皩Ｉ尽惫ぷ黝I(lǐng)導(dǎo)小組全面負(fù)責(zé)“專升本”工作，領(lǐng)導(dǎo)小組下設(shè)辦公室。

二、報(bào)考對象

2015屆（2012級(jí)）臨床醫(yī)學(xué)（含婦幼保健方向、醫(yī)學(xué)影像方向）、醫(yī)學(xué)檢驗(yàn)技術(shù)、醫(yī)學(xué)影像技術(shù)、護(hù)理（含涉外護(hù)理方向、社區(qū)護(hù)理方向）、助產(chǎn)、口腔醫(yī)學(xué)、針灸推拿、藥學(xué)、醫(yī)藥營銷等13個(gè)專業(yè)及方向普通?？茟?yīng)屆畢業(yè)生。

三、報(bào)考條件

1、德智體全面發(fā)展，在校期間無嚴(yán)重違紀(jì)現(xiàn)象。

2、各科成績合格，考試科目總分進(jìn)入本專業(yè)同年級(jí)前20%。

3、非英語專業(yè)（音、體、美專業(yè)除外）學(xué)生必須在進(jìn)入?？齐A段就讀并取得有效學(xué)籍之后，至2015年3月底以前獲得《高等學(xué)校英語應(yīng)用能力考試》（A級(jí)）證書或英語四級(jí)考試成績達(dá)到426分以上。

推薦參加“專升本”選拔考試的學(xué)生需同時(shí)滿足以上條件。

4、應(yīng)征入伍的在校?？粕诓筷?duì)榮獲三等功的，退役后辦理了復(fù)學(xué)手續(xù)并完成了?？齐A段學(xué)習(xí)、取得《高等學(xué)校英語應(yīng)用能力考試》（A級(jí)）證書或四級(jí)考試成績達(dá)到426分以上的應(yīng)屆畢業(yè)生，可免試進(jìn)入本科階段學(xué)習(xí)。普通應(yīng)屆畢業(yè)生服義務(wù)兵役退役后，要求“專升本”的，由學(xué)生本人持相關(guān)證件于3月1日－20日（公休日除外）到省大中專學(xué)校學(xué)生信息咨詢與就業(yè)指導(dǎo)中心報(bào)名，按湖南省教育廳《關(guān)于印發(fā)<普通高職高專畢業(yè)生服義務(wù)兵役退役后接受普通本科教育實(shí)施辦法（試行）>的通知》（湘教通〔2010〕479號(hào)）和本實(shí)施辦法的有關(guān)規(guī)定執(zhí)行。

5、有下列情形之一者，不能報(bào)考：（1）未交清學(xué)校所有相關(guān)費(fèi)用者。（2）在校期間受過處分，尚未撤消者。

四、報(bào)考事項(xiàng)

1、根據(jù)湘教發(fā)〔2015〕4號(hào)關(guān)于印發(fā)《湖南省普通高等教育 “專升本”工作實(shí)施辦法》的通知的文件精神，今年學(xué)校嚴(yán)格按通過審核的名單組織考試?？忌Y格審查與選拔程序嚴(yán)格按照《湖南醫(yī)藥學(xué)院“專升本”管理辦法》執(zhí)行。

2、報(bào)考的專業(yè)必須與本人在?？齐A段學(xué)習(xí)的專業(yè)相同或相近。

3、報(bào)名費(fèi)：400元/人。

4、報(bào)名時(shí)間：2015年3月26日～4月9日。5、4月15日～4月22日公示參加考試人員名單。

五、擬報(bào)考學(xué)校

1、中醫(yī)藥大學(xué)：口腔醫(yī)學(xué)專業(yè)

2、湘南學(xué)院：臨床醫(yī)學(xué)（含婦幼保健方向、醫(yī)學(xué)影像方向）、護(hù)理（含涉外護(hù)理方向、社區(qū)護(hù)理方向）、助產(chǎn)、醫(yī)學(xué)影像技術(shù)、醫(yī)學(xué)檢驗(yàn)技術(shù)、藥學(xué)等專業(yè)

3、吉首大學(xué)：針灸推拿專業(yè)、醫(yī)藥營銷專業(yè)

六、擬考試課程 待定。

七、考試時(shí)間、地點(diǎn) 待定。

八、工作要求

1.各系要進(jìn)行廣泛宣傳發(fā)動(dòng)，要將通知精神告知2012級(jí)三年制高專注冊的所有學(xué)生，并在2013、2014級(jí)學(xué)生中傳達(dá)，以激發(fā)學(xué)生的學(xué)習(xí)熱情，端正學(xué)風(fēng)、嚴(yán)肅考風(fēng)。

2.嚴(yán)格堅(jiān)持公開、公平、公正、擇優(yōu)的原則。

3.嚴(yán)格把關(guān)，嚴(yán)禁推薦過程中弄虛作假，行賄受賄，一經(jīng)查出，學(xué)生立即取消報(bào)考資格，有關(guān)人員給予嚴(yán)肅處理。

備注：報(bào)考具體相關(guān)事項(xiàng)及流程、擬報(bào)考課程、考試時(shí)間和地點(diǎn)將另行通知。

根據(jù)湖南省教育廳《關(guān)于做好2012年高等學(xué)校普通專

科畢業(yè)生“專升本”工作的通知》（湘教通?2011?461號(hào)）精神，學(xué)校決定繼續(xù)在?？茟?yīng)屆畢業(yè)生中選拔優(yōu)秀學(xué)生進(jìn)入本科三年級(jí)學(xué)習(xí)，在本科階段修滿兩年，成績合格畢業(yè)，獲得本科文憑。為切實(shí)做好我校2012年“專升本”工作，現(xiàn)將有關(guān)事項(xiàng)通知如下。

一、加強(qiáng)“專升本”工作的組織領(lǐng)導(dǎo)

為做好2012年 “專升本”工作，學(xué)校成立“專升本”

工作領(lǐng)導(dǎo)小組，組長由魏飴校長擔(dān)任，副組長由校紀(jì)委李敏書記、龔作奇副校長擔(dān)任，成員由教務(wù)處、學(xué)工部、紀(jì)委監(jiān)察處等部門負(fù)責(zé)人組成。

二、選拔范圍和比例

1.范圍：我校2012屆普通?？茟?yīng)屆畢業(yè)生。

2.比例：推薦參加“專升本”選拔考試的人數(shù)為選拔范圍內(nèi)各專業(yè)學(xué)生數(shù)的20%，選拔注冊比例控制在參加選拔考試學(xué)生總數(shù)的50%以內(nèi)（按升入的本科專業(yè)分別計(jì)算）。“專生本”招生計(jì)劃由學(xué)校申請，教育廳核定。

三、推薦條件

1.德、智、體全面發(fā)展。

2.各科成績平均分(補(bǔ)考科目按60分計(jì)算)進(jìn)入本專業(yè)2012屆學(xué)生的前20%。

3.非英語專業(yè)(音、體、美專業(yè)除外)學(xué)生在校期間（取得有效學(xué)籍至2012年3月底以前）必須獲得《高等學(xué)校英語應(yīng)用能力考試》（A級(jí)）及以上等級(jí)證書（或大學(xué)英語四級(jí)成績達(dá)到426分以上）。

推薦參加“專升本”選拔考試的學(xué)生需同時(shí)滿足以上條件。省級(jí)三好學(xué)生、三好學(xué)生標(biāo)兵、優(yōu)秀學(xué)生干部以及在省級(jí)各類競賽中獲得等級(jí)獎(jiǎng)的學(xué)生，在同等條件下予以優(yōu)先考慮。凡德育綜合測評不合格、體育測試不合格、受到記過以上處分后無突出表現(xiàn)者，一票否決。

4.應(yīng)征入伍的在校?？粕诓筷?duì)榮獲三等功的，退役后辦理了復(fù)學(xué)手續(xù)并完成了?？齐A段學(xué)習(xí)、取得《高等學(xué)校英語應(yīng)用能力考試》（A級(jí)）證書或英語四級(jí)考試成績達(dá)到426分以上的應(yīng)屆畢業(yè)生，可免試進(jìn)入本科階段學(xué)習(xí)。

四、選拔方式與步驟

1.學(xué)生本人提出申請。

2.相關(guān)學(xué)院推薦。

（1）在學(xué)生自愿申請的基礎(chǔ)上，相關(guān)學(xué)院嚴(yán)格按推薦條件選拔成績排名在本專業(yè)學(xué)生總數(shù)前20％范圍的2012屆畢業(yè)生作為推薦對象。

（2）各相關(guān)學(xué)院于2012年3月20日前將被推薦學(xué)生名單匯總后連同英語等級(jí)考試證書原件或大學(xué)英語四級(jí)成績單交教務(wù)處學(xué)籍科，經(jīng)學(xué)校審核后，從2012年3月 28日起張榜公布7天，如無異議，即組織被推薦學(xué)生填寫“專升本”志愿表，每位學(xué)生限填報(bào)一個(gè)對口專業(yè)。學(xué)校于4月15日前報(bào)省教育廳審查。

3.學(xué)校組織選拔考試。

凡填報(bào)我校志愿的本?；蛲庑．厴I(yè)生，均須參加由我校統(tǒng)一組織的選拔考試，考試日期暫定為2012年5 月5日。

4.確定選拔名單。

學(xué)?！皩Ｉ尽惫ぷ黝I(lǐng)導(dǎo)小組根據(jù)選拔考試成績，結(jié)合學(xué)院推薦意見和學(xué)生平時(shí)綜合表現(xiàn)情況，按推薦報(bào)考本科專業(yè)學(xué)生的50％的比例和不超過教育廳下達(dá)我校的“專升本”計(jì)劃數(shù)提出選拔名單。擬選拔對象于5月底前張榜公布，若無異議，學(xué)校于6月上旬將選拔名單報(bào)省教育廳審核注冊。正式“專升本”學(xué)生以省教育廳批準(zhǔn)文件為準(zhǔn)，學(xué)生接到“專升本”錄取通知書后，即可憑通知書辦理有關(guān)手續(xù)。

五、其它事項(xiàng)

1.有關(guān)“專升本”的推薦、考試和選拔工作將按校務(wù)公開的要求，實(shí)行全程公開。學(xué)校紀(jì)檢監(jiān)察部門將對“專升本”工作實(shí)行全程監(jiān)督，確保公平、公正、公開，對徇私舞弊、違規(guī)操作的將嚴(yán)肅處理。

根據(jù)河南省教育廳《關(guān)于選拔2014年普通高等學(xué)校優(yōu)秀?？飘厴I(yè)生進(jìn)入本科階段學(xué)習(xí)的通知》（教學(xué)【2014】168號(hào)）文件要求，結(jié)合我院2014年畢業(yè)生實(shí)際情況，請各院系組織預(yù)參加專升本的學(xué)生進(jìn)行確認(rèn)及信息采集，推薦比例不得超過本專業(yè)畢業(yè)生的25%。按照《教育部辦公廳關(guān)于做好普通高職（?？疲┊厴I(yè)生服義務(wù)兵役退役和“下基層”服務(wù)期滿后接受本科教育招生工作的通知》（教學(xué)廳〔2009〕6號(hào)）要求，2010年后退伍的且具有河南交通職業(yè)技術(shù)學(xué)院普通?？茖W(xué)歷的退役士兵也可在原就讀院系參加2014年專升本考試報(bào)名，報(bào)名人數(shù)不受專業(yè)報(bào)名比例限制。報(bào)名時(shí)，退役士兵需提交畢業(yè)證書和士兵退役證?，F(xiàn)就專升本學(xué)生基本信息采集工作安排如下：

一、基本信息采集時(shí)間安排

（一）各院系于3月17日-19日通知本院系預(yù)參加專升本的學(xué)生到輔導(dǎo)員老師處填寫《學(xué)院2014年專升本推薦考生基本信息表》（附件3），3月19日18：00前交招生就業(yè)處匯總、審核。

（二）經(jīng)學(xué)院審核通過的“專升本”推薦考生名單，3月20日反饋各院系，并請各院系按要求公示三日。

二、基本信息錄入時(shí)間安排

各院系組織專人按《學(xué)院2014年專升本推薦考生基本信息表》（原

就讀?？茖I(yè)排序）在本院系進(jìn)行推薦考生的信息輸入，時(shí)間從3月20日至23日（采集程序隨文同時(shí)下發(fā)，使用方法見附件4，報(bào)名序號(hào)由學(xué)院統(tǒng)一編排）。推薦表各院系負(fù)責(zé)打印，一式二份，由學(xué)生本人核對無誤后，貼上照片（2寸藍(lán)底證件照），并簽字確認(rèn)，各院系匯總收齊（按報(bào)考專業(yè)代號(hào)、報(bào)名序號(hào)由小到大排序，每名學(xué)生的2份推薦表放在一起）于3月24日12：00前交回郭老師處。

請各院系嚴(yán)格按照此時(shí)間安排，逾期將不予辦理。需考生本人簽字確認(rèn)的信息，任何人不得代簽。

三、要求

（一）各院系接到通知后，要及時(shí)將有關(guān)精神向全體學(xué)生傳達(dá)，做好組織宣傳工作，確保將真正優(yōu)秀的學(xué)生推薦到本科階段學(xué)習(xí)。

（二）所有被推薦學(xué)生從即日起必須在鄭，各院系要派專人負(fù)責(zé)，以便及時(shí)核對個(gè)人信息。負(fù)責(zé)人名單及聯(lián)系方式隨專升本信息采集表同時(shí)上報(bào)招生就業(yè)處。

（一）報(bào)考條件: 根據(jù)文件規(guī)定，湖南工程學(xué)院專升本，本次考試選拔對象，應(yīng)符合以下條件：
