      船舶和設(shè)備的維護程序 Ship and Equipment Maintenance Procedure

      1.目的Objective 本程序旨在防止船舶和設(shè)備得不到良好或有效的維護而喪失其安全與環(huán)境保護功能的要求,也防止公司將不合要求的船舶和設(shè)備投入海上營運,確保船舶和設(shè)備處于適航與適貨狀態(tài)。

      This procedure aims at keeping good and effective maintenance of ships in conformity with requirement of safety and environmental protection, also aims at preventing ships not comply with requirement of seaworthy/ cargoworthiness put into operation.2.適用范圍Applicability 本程序適用于船舶和設(shè)備的維護保養(yǎng)。

      This procedure shall apply to maintenance of ship and equipments.3.職責(zé)

      Responsibilities 3.1.機務(wù)部

      Technical Department 3.1.1.機電主管部門,有責(zé)任使船舶和設(shè)備處于適航狀態(tài)并滿足主管機關(guān)、船檢機構(gòu)的要求以及公司方針目標(biāo)的實現(xiàn)。

      Technical Department is responsible for maintenance of ship and complies with requirement of administrations, class society and company regulations.3.1.2.負(fù)責(zé)制定船舶設(shè)備維護周期和分工明細(xì)表,并檢查、督促和指導(dǎo)船舶制定、實施船舶及設(shè)備維護計劃;

      To be responsible for drawing up the ship?s annual maintenance plan, to be responsible for supervising of the drawing and carrying out of maintenance of ships and equipments and providing enough guidance.3.1.3.負(fù)責(zé)船舶備件、物料的審批、供應(yīng)、管理,審閱、處理船舶上報的各類有關(guān)船舶及設(shè)備維護方面的技術(shù)報告及報表。

      To be responsible for approval, supplying, management of ship?s spare parts and materials;to be responsible for the vetting, filing of the technical reports and relevant records reported from the ship which regarding ship and equipment maintenance.3.2.船舶

      Ships 3.2.1.船長和船員是實施維護保養(yǎng)的主體,以保持船舶和設(shè)備符合規(guī)定的船級要求;

      The master and crewmember is mainstay of ship maintenance so as to keep ship and equipment complies with requirement of class.3.2.2.負(fù)責(zé)船舶和設(shè)備維護計劃的制定和實施以及確保進行規(guī)定的檢查;

      To be responsible for planning and carrying out ship maintenance plan, ensure inspecting according to regulations.3.2.3.負(fù)責(zé)船舶物料、備件的使用管理;

      Responsible for the using of ship?s stores, spare parts management.3.2.4.負(fù)責(zé)向機務(wù)部報告船舶和設(shè)備的缺陷并向指定人員報告船舶和設(shè)備的不符合規(guī)定情況。

      Responsible for reporting defects of ship and equipments to Technical Department and reporting non-conformities situations to DP.4.工作程序

      Working Procedure 4.1.制定預(yù)防維護計劃 Drawing up of maintenance plan 4.1.1.機務(wù)部應(yīng)根據(jù)船舶設(shè)備維護周期和分工規(guī)定的養(yǎng)護周期,并結(jié)合有關(guān)規(guī)范、規(guī)則要求、船檢機構(gòu)的要求、設(shè)備說明書、船舶現(xiàn)狀及以往設(shè)備故障和缺陷的處理情況等,在每年十二月初編寫下年度的維護計劃,下達(dá)船舶執(zhí)行。

      Technical Department should draw up ship?s Annual Maintenance Plan for next year every December according to maintenance intervals and requirements of relevant international conventions/ Flag State / class / Instructions of Manufacture and the Ship?s current conditions/dispose situation of former equipment failure and defects etc, and sent to ship for carrying out.4.1.2.該計劃應(yīng)特別規(guī)定標(biāo)識和保障對船舶因突發(fā)性運行故障而導(dǎo)致險情的設(shè)備和技術(shù)系統(tǒng)以及備用設(shè)備和長期擱置裝置的維護與保養(yǎng)。

      The plan should specially provide identification and ensuring of maintenance about equipments have suddenly operation troubles and result in dangerous/technique system/standby equipment/equipments lie idle for long-term.4.2.預(yù)防維護計劃的實施

      4.2.1.船長和船舶各部門長應(yīng)根據(jù)船舶設(shè)備維護周期和分工,按年度維護計劃中的維護項目每月組織船員開展維護檢修工作,并由檢修責(zé)任人將維護工作做好記錄。

      The master and the Department Chief should assign the tasks to the respective personnel and organize the ship?s maintenance as per the maintenance plan and periodic table;the responsible staff should record the maintenance work.4.2.2.在實施過程中,養(yǎng)護人員應(yīng)遵守船舶輪機設(shè)備維護的相關(guān)規(guī)定。檢修責(zé)任人必須將負(fù)責(zé)的維護項目進行認(rèn)真檢查、驗收,確保維護檢修計劃的有效實施。

      During the period of implementation the maintenance person must comply with relevant provisions and the responsible person must exercise checking and confirmation for the maintenance items so as to ensure that the maintenance plan be completed with quality and quantity guarantee.4.2.3.船舶各部門長在每月底將養(yǎng)護工作的實施情況分別填寫月度維修保養(yǎng)計劃表,由船長審核簽字后上報機務(wù)部主管。

      Department chief is responsible for recording implementation situations and filling in < Monthly Maintenance Plan>, master should examine and signed and then submit to technical manager.4.2.4.對本月未能完成的維護項目,部門長應(yīng)將其記錄在各部門月度維護檢修工作報表中說明原因,并列入下月維護計劃。

      Department chief should write the unfinished items in monthly maintenance plan of each department and explain reasons, then list them in plan of next month.4.3.船舶修理 Ship repair 4.3.1.船舶修理類別 Sort of ship repair 分為航修、計劃修理(包括檢修)和事故修理。Voyage repair, scheduled repair, accident repair 4.3.1.1.航修 Voyage repair 航修屬臨時性修理,不編計劃。主要是為了解決營運中發(fā)生的局部故障,影響航行安全,而船舶又不能自修的工程。航修前由船長簽署各部門編寫的“航修報告”報機務(wù)部評審后通知船舶安排修理。航修可由航修廠利用船舶在港期間進行,不影響船舶營運。修理過程中,船上相關(guān)人員應(yīng)按規(guī)定做好監(jiān)修和驗收工作。

      Voyage repair is temporality repair and not included in repair plan.That aims at settling partial failure which occurred during operation and affect safety of sailing but can?t be repaired by crewmember.Technical department would review the “voyage repair report” signed by master and inform ship about repair arrangement.Voyage repair could be arranged when ship is in port in order to avoid loss of time.Relevant person should be supervising and inspection the repair work during repairing.4.3.1.2.計劃修理(分小修和檢修)Scheduled Repair(minor repair and overhaul)4.3.1.2.1.小修:按規(guī)定周期有計劃地結(jié)合“期間檢驗”或“年度檢驗”進行的廠修工程。一般每12—18個月為一個周期。

      Minor repair: Repair works carried out in shipyard refer to period and combine with “period survey” or “annual survey”.One period lasts for 12 to 18 months.4.3.1.2.2.檢修:按規(guī)定周期每隔2—3次小修并結(jié)合“定期檢驗”或“特別檢驗”進行的廠修工程。一般每5年為一個周期。

      Overhaul: Repair works carried out in shipyard every 2-3 Minor repair and combine with “periodical survey” or “special survey” 4.3.1.2.3.事故修理:船舶發(fā)生事故后,應(yīng)根據(jù)船舶損壞情況和檢驗部門提出的修理范圍和要求進行。如果通過臨時修理可以取得適航證書的可做臨時性修理,以減少營運損失。如損壞嚴(yán)重則應(yīng)根據(jù)當(dāng)時當(dāng)?shù)氐臈l件決定修理方案。事故修理如距計劃修理時間較近可以考慮合并進行。重大事故的修理,公司應(yīng)派機務(wù)經(jīng)理進行監(jiān)修。

      Accident repair: repair work should be carried out according to damages of ship and requirement of relevant survey department once there was one accident.In order to reduce loss of hire temporary repair should be done if that can meet the requirement of seaworthy.Repair plan should be drawn up according to condition of then and there if there is a serious damage.If the accident repair is close to scheduled repair, combining could be considered.A superintendent is need once there was a grave accident repair.4.3.2.修船準(zhǔn)備 Prepare work for ship repair 4.3.2.1.編制修理計劃 Drawing up repair plan 4.3.2.1.1.船舶修理計劃分年度、月度修理計劃二種。由機務(wù)部經(jīng)理根據(jù)有關(guān)規(guī)范、規(guī)則要求,船檢證書和檢驗類別,設(shè)備制造廠的要求,近年來的修理記錄,船舶現(xiàn)狀及船舶關(guān)于設(shè)備的失效/損壞、故障和缺陷的反饋信息,在修理年度前的年底編制年度修理計劃,報送總經(jīng)理,審批下達(dá)。

      There are two kinds of repair plan, annual plan and monthly plan.Manager of Technical Department should draw up ship?s Annual Maintenance Plan for next year at the end of every year according to maintenance intervals and requirements of relevant international conventions/ ship survey certificates and sort / requirement of Manufacture /records of repair and the Ship?s current conditions/dispose situation of former equipment failure and defects etc, and submit to general manager of approval, then make known to lower levels.4.3.2.1.2.月度修理計劃根據(jù)船舶營運安排和年度計劃以及船舶實際狀況,由機務(wù)部經(jīng)理按年度修理計劃中的進廠日期應(yīng)提前三個月編妥,主要項目為船名、修理類別、計劃修期、修船廠名、計劃進廠時間,以便于和船舶及船廠聯(lián)系。

      Manager of Technical Department should draw up a monthly repair plan three months earlier then the date of going into shipyard which listed in annual plan according to ship?s operation, annual plan and ship?s actual condition.Main items should include vessel name, sort of repair, planed period of repair, name of shipyard, planed time going into shipyard so as to keep good communication with ship and shipyard.4.3.2.2.編制修理項目工程單 Draw up EO-of repair

      4.3.2.2.1.船舶各部門長應(yīng)根據(jù)年度修理計劃和月度修理計劃,在廠修前(2-3)個月編寫修理單,由船長審閱后報機務(wù)部經(jīng)理審批。修理單可按塢修、甲板、輪機、電氣四個部份編制。

      Department chief on board should draw up repair list 2-3 months earlier then shop repair according to annual repair plan and monthly repair plan, then sent to manager of technical manager for approval by master.Repair list could be draw up by four sections: docking, deck, and engine, electric.4.3.2.2.2.機務(wù)部收到修理單后,機務(wù)經(jīng)理應(yīng)上船審查核對,并進行必要的補充和修正,最后修理單的項目由機務(wù)部負(fù)責(zé)定案,船員如有不同意見,應(yīng)主動介紹情況,說明理由,通過協(xié)商,由機務(wù)部最后決定。

      Once received the repair list, manager of technical department should go onboard for check and examine, make the necessary supplement and correction, the repair list should be finalized by technical department, person onboard should brief and adduce if he has different opinions, then technical department finally decided after discussion.4.3.2.2.3.事故修理由船舶編制修理項目工程單報機務(wù)部審核實施。

      Refer to accident repair, ship should draw up EO-of repair and submit to technical department for approval and then implement the list.4.3.2.3.進廠準(zhǔn)備prepared into shipyard 4.3.2.3.1.船舶進廠前應(yīng)準(zhǔn)備好修船必需備件。重大部件至少應(yīng)在半年前提出申請,以便及時訂貨。對于需要的大量材料及特殊材料,在船廠選擇定后,應(yīng)提前通知船廠。

      Spares should be prepared before ship enters the shipyard.Big spares should be applied for at least 6 months advanced so as to order timely.If mass materials or special materials are needed, an advanced notice should be given to the selected shipyard.4.3.2.3.2.為了便于船廠做好修船準(zhǔn)備,應(yīng)盡可能安排船廠人員上船看工程,以便爭取船期,船舶進廠前,船長應(yīng)組織各部門長,統(tǒng)一指揮修船工作,研究處理修船中的技術(shù)業(yè)務(wù)問題。

      It would be better to arrange the worker of shipyard go on board to check the repair project so as to save repair time.Master should organize every department chief, command the repair work in unify and deal with technical problem during repair.4.3.2.3.3.進廠前應(yīng)備妥施工可能使用的圖紙資料、專用工具、清理影響施工的物品、危險品,為施工提供安全工作條件。

      Information and drawing, special tools should be prepared before shop repair, obstacle and object would affect construction should be cleared and provide safe working conditions.4.3.2.3.4.船舶修理范圍涉及主機、錨機、舵機等工程,如廠方要求可在修理前進行預(yù)試航。預(yù)試航由船舶、機務(wù)、修方各派員參加。

      Ship repair project includes main engine, windlass, and steering engine;trial trip could be carried out if required by shipyard.Ships, technical department, shipyard should design person to attend the trial trip.4.3.2.4.增減工程 Increase and decrease of project 4.3.2.4.1.船舶進廠后,根據(jù)檢查的情況允許對工程范圍內(nèi)的項目進行修正。但增減工程項目范圍必須嚴(yán)格控制,在修船過程中,允許在修理合同三分之一的修期內(nèi)增減項目,以免影響工程進度。

      Items of repair project could be revised refer to result of checking when vessel in shipyard.The increase or decrease range should be strictly controlled, In order to avoid impact progress of works, only changes before former one-third of whole time is permitted when vessel is under repair.4.3.2.4.2.增減工程項目應(yīng)由船長與機務(wù)經(jīng)理決定。

      Increase or decrease of project should be decided by master and manager of technical manager.4.3.2.5.監(jiān)修和驗收

      Supervision and accepting

      船舶進廠修理時,公司應(yīng)派出機務(wù)經(jīng)理或主管,負(fù)責(zé)與船廠聯(lián)系,決定修理工程,處理修船中發(fā)生的問題。船舶應(yīng)負(fù)責(zé)具體修理項目的監(jiān)修和驗收工作。

      The company should appoint manager of technical department or person in charge to be responsible for keeping communication with shipyard, decide repair project and deal with problems during the repair.Ship to be responsible for supervising, examining and receipting the repair work.4.3.2.5.1.監(jiān)修

      Supervising 4.3.2.5.1.1.一般工程由輪機長和大副分別組織人員監(jiān)修,重大工程應(yīng)由輪機長和大副親自監(jiān)修。

      Chief engineer or chief officer should supervise grave projects by themselves and appoint person to supervise general projects;4.3.2.5.1.2.監(jiān)修人負(fù)責(zé)監(jiān)督船廠是否按修理單指定的范圍和要求施工;

      Superintendent is responsible for supervise whether the shipyard construct according to requirement of repair list.4.3.2.5.1.3.工藝、材料及安裝質(zhì)量是否符合技術(shù)要求;

      Whether technology, quality of material and installing are pass muster.4.3.2.5.1.4.施工中有無船廠責(zé)任引起的部件及設(shè)備的損壞;

      Whether any damage of spare or equipment caused by responsibility of shipyard.4.3.2.5.1.5.施工中有無不安全因素,可能引起火災(zāi)及其它危險,必要時有權(quán)停止其施工,并向主管人員匯報,等待處理。

      Is there any factor of unsafely which may cause a fire or other dangerous, superintendent is authorized to stop construction as necessary, and then report to people in charge for settling.4.3.2.5.1.6.做好必要的修理記錄,以便對驗收和審核帳單準(zhǔn)備材料;

      Make necessary repairs records so as to get prepared for accepting and checking debit note accordingly.4.3.2.5.1.7.監(jiān)修人應(yīng)配合船廠工作,為施工提供方便條件。

      Superintendent should coordinate with the shipyard and provide convenience for repair project.4.3.2.5.2.驗收

      Acceptance 4.3.2.5.2.1.施工過程中,船員應(yīng)切實了解工程進度和做好質(zhì)量驗收工作。船廠施工完畢應(yīng)交船員驗收,驗收時應(yīng)由船、廠雙方代表在場,驗收后由驗收人簽字,作為該項工程的結(jié)束。所有船檢要求檢驗的項目,必須督促廠方按期申請交驗。除船檢規(guī)定要求檢驗項目外,為確保修船質(zhì)量,船方的質(zhì)量驗收應(yīng)做到:

      Crewmember should follow up the progress of works and get prepared for acceptance.The shipyard should ask crewmember to examine and accept the project with representatives of both ship and shipyard be present when they finish repairing work, then person in charge of accept signed for acceptance and then the work over.Refer to items required by ship inspection department should be finished on time.Besides items required by ship inspect department, in order to ensure qualify of repair work, qualify acceptance should include:

      4.3.2.5.2.2.一般修理項目,需船方部門負(fù)責(zé)人檢查滿意

      General repair item should be examined and accepted by department chief.4.3.2.5.2.3.水線以下工程的施工安裝、驗收,大副、輪機長必須親臨現(xiàn)場,必要時機務(wù)經(jīng)理或主管參加驗收。

      Project below waterline should be supervised and accepted by chief officer and/or chief engineer, manager of technical department or person in charge should attend for acceptance if necessary.4.3.2.5.2.4.對影響航行的重要機電設(shè)備,如主機、發(fā)電機、舵機、錨機等的修理,從拆下、解體、加工、安裝、測試、試車等過程,船舶主管人員必須到現(xiàn)場共同檢查,機務(wù)經(jīng)理或主管須隨時到現(xiàn)場掌握修理質(zhì)量的動態(tài)。

      Refer to repair of those important electromechanical equipments which affect safety of sailing, such as main engine, generator, steering engine and windlass etc, governors onboard should examine the whole process include demounting, disassembly, machining, installing, testing, and trial run, technical manager or person in charge should attend the sense and master process/qualify at any time.4.3.2.5.2.5.廠方必須準(zhǔn)確、及時提交修理船舶質(zhì)量檢驗書面報告,一份交機務(wù)部,一份交船。

      The shipyard should submit exact Repair Quality Inspection Report to Technical Department and ship.4.3.2.5.2.6.單項工程完畢后,可進行單項驗收。需要運轉(zhuǎn)試驗的設(shè)備,應(yīng)進行運轉(zhuǎn)試驗。必須試航才能驗收的項目,可留待試航時驗收。凡不符合要求的項目,應(yīng)由船廠負(fù)責(zé)解決,雙方如有爭議,首先進行協(xié)商,如協(xié)商不成,以船檢部門的要求為準(zhǔn)。

      Single project could be examined and accepted separately once finished.Equipment should be tested run if needed.Items needed to be examined during trial trip should be waiting for trialing trip.If any undesirable item, the shipyard should be responsible for rectification, any dispute should be consulted firstly and if fails, requirement of ship inspection society should be applicable.4.3.2.5.2.7.修理完工后可根據(jù)修理范圍決定是否需要試航,或在碼頭試車。試航時應(yīng)由雙方提出試航大綱,明確試航時的安全責(zé)任。試航中發(fā)現(xiàn)的問題,凡應(yīng)廠方負(fù)責(zé)的項目應(yīng)由船廠負(fù)責(zé)修理。

      Whether a trial trip or test run is needed should be refer to extent of scope of repair.Compendium of trial trip should be done by both two sides and clear safety responsibility in trial trip.Problems found in trial trip should be settled by shipyard in their responsibility range.4.3.2.5.2.8.修理完畢,應(yīng)組織力量,認(rèn)真審核完工單。完工單是編制帳單的主要依據(jù),要嚴(yán)格把關(guān),屬于質(zhì)量未達(dá)到要求的應(yīng)文字注明,并雙方簽字。

      The Completion List should be checked carefully and strictly when repair is finished for that?s main basis of preparing the debit note.Unqualified repair work should be marked out by text notation and signed by both side.4.3.2.6.船舶自修

      Self-repair onboard 船舶自修分為營運期間的自修和廠修期間的自修兩種。

      Self-repair includes repair during operation and repair in shipyard.4.3.2.6.1.船舶營運期間,船員在完成預(yù)防檢修計劃的前提下,可以自修屬于廠修范圍的工程,一般不占用營運時間,但對運輸任務(wù)繁忙、停泊時間短,或船舶定員偏少的船舶,可由公司給予一定的停航檢修時間,必要時可申請船廠、航修廠協(xié)助。

      When ship is under service, crew member could do some repair works in premise that they finished the prophylactic repair plan, which always don?t occupy time of operation.But to that ship which is under busy service, short anchor time or have less person onboard, the company should give some time for repair suspend service, help of shipyard should be arranged if necessary.4.3.2.6.2.船舶廠修期間,船員應(yīng)盡可能多地做一些屬于廠修范圍的自修工程,進廠前應(yīng)在編制修船計劃的同時編制自修計劃,由機務(wù)部批準(zhǔn)后執(zhí)行。自修計劃應(yīng)與廠修相配合,按廠修預(yù)計時間,除監(jiān)修及日常工作外的人員,能力統(tǒng)籌考慮。

      Crew members should do some self-repair works as more as possible when vessel is under repairing in shipyard.A self-repair plan should be drawn up together with repair plan before ship enters shipyard and implemented when approved by Technical Department.Self-repair plan should comply with time of shop repair, and make competitive arrangement about labor power except superintendent and person in daily duty.4.3.2.6.3.自修、廠修工程應(yīng)注意不要互相干擾,自修工程應(yīng)與廠修同時完工,不影響開航。Self-repair and shop repair shouldn?t in mutual interference, and should be finished at the same time so as to set sail on time.4.3.2.6.4.自修工程應(yīng)嚴(yán)格控制質(zhì)量,工程完畢后由船舶領(lǐng)導(dǎo)負(fù)責(zé)驗收。對重大的自修項目,應(yīng)請機務(wù)經(jīng)理或主管親自驗收。

      The quality of self-repair should be strictly controlled, and leader onboard should check and accept the repair work themselves.Refer to important items must be accepted by technical manager or person in charge.4.3.2.7.安全保衛(wèi)

      Security 4.3.2.7.1.進廠后船舶領(lǐng)導(dǎo)應(yīng)及時與船廠聯(lián)系,了解船廠的安全保衛(wèi)制度,向船廠說明有關(guān)船舶安全保衛(wèi)的要求,共同落實防火、防風(fēng)、防污染等各項安全措施,并簽訂“安全保衛(wèi)協(xié)議”。

      Leader onboard should contact with shipyard timely once ship entered shipyard, inquiry security system of shipyard and state security requirement of the ship, carry out each safety measure jointly such as fire prevention, anti-pollution etc, and sign ?Agreement of Security?.4.3.2.7.2.在廠期間的抗災(zāi)施救工作應(yīng)服從船廠的統(tǒng)一指揮,但應(yīng)積極做好各項安全準(zhǔn)備工作,一旦發(fā)生災(zāi)害性事故,全船應(yīng)堅守崗位,奮力搶救,保衛(wèi)船舶和人員的安全。

      Disaster rescue should follows shipyard?s unified command when vessel in shipyard, but the ship should get prepared for security, once there is a disaster accident, the whole ship should hold position, struggle for the rescue, and defend safety of vessel and crew.4.3.2.8.修船總結(jié)

      summary of repair 4.3.2.8.1.修船結(jié)束后,船長應(yīng)及時組織各部門人員進行總結(jié),在一個月內(nèi)向機務(wù)部寫出書面修船總結(jié)報告??偨Y(jié)報告包括的內(nèi)容:

      Master should organize every department for summarizing and submit a writer report to Technical Department in one month form date of finishing.The report should include: 4.3.2.8.2.修船地點、時間、修理廠、修理類別; Place/time/shipyard/sort of repair 4.3.2.8.3.修理主要項目,修理質(zhì)量,對船廠工作的評價及自修工作情況;

      Main repair items, quality of repair, evaluation about work of shipyard and status of self-repair.4.3.2.8.4.修船遺留的問題及以后修理注意的事項和建議; Problem left over, items should be noticed in future and advices.4.3.2.8.5.附修理的檢查測量記錄及其它技術(shù)檔案資料,以便機務(wù)部審閱后將其作為船舶單船技術(shù)檔案歸檔。

      Check measurement record and other relevant technical materials should be submit to Technical Department for check and approve and then filed.4.3.2.9.修船質(zhì)量反饋 Repair quality feedback

      Check on board 4.5.1.1.船長應(yīng)檢查、督促船舶各部門按檢修計劃對船舶和設(shè)備實施有效的維護。Master should exercise checking and supervising every department onboard implementing of repair schedule on board so as to ensure the ship and equipments maintained effectively.4.5.1.2.船舶開航前檢查按相關(guān)的規(guī)定執(zhí)行。

      Inspection of company?s relevant function department should refer to < Ship Safety Inspection System >.4.6.報告和實施糾正措施 Reporting and implementing corrective action 4.6.1.船舶在維護和在檢查中發(fā)現(xiàn)的設(shè)備故障或缺陷,如船上有能力自行修理解決,應(yīng)迅速予以糾正,并將糾正情況記錄在船舶和設(shè)備維護報告表報機務(wù)部。

      —月度預(yù)防檢修計劃。責(zé)任 4.1 船長

      ??對船舶和設(shè)備預(yù)防檢修計劃的執(zhí)行情況負(fù)責(zé)。??每月要對各部門上報的設(shè)備養(yǎng)護執(zhí)行記錄簽字確認(rèn)。??負(fù)責(zé)向公司報告船舶及設(shè)備上的缺陷,并落實公司的指令,負(fù)責(zé)落實缺陷的糾正。4.2 船舶部門負(fù)責(zé)人(大副、輪機長)

      ??負(fù)責(zé)主管設(shè)備的維護保養(yǎng),保證設(shè)備處于良好的工作狀態(tài)。??領(lǐng)導(dǎo)、指導(dǎo)養(yǎng)護人員對主管設(shè)備按規(guī)定正確保養(yǎng)、維修。??對所管設(shè)備的日常維修、檢修質(zhì)量負(fù)責(zé)檢查和驗收。所有維修保養(yǎng)情況要記錄在《檢修工作記錄簿》中。??記錄各項測量數(shù)據(jù)和報表,并按規(guī)定送部門長審核上報。??負(fù)責(zé)主管的船舶及設(shè)備缺陷分析原因,并向部門長報告,按照公司指令落實糾正措施。4.4 各船舶的機務(wù)主管

      ??審核船舶甲板、輪機預(yù)防檢修計劃及每月執(zhí)行情況; ??負(fù)責(zé)落實修理安排、檢查修理安全和質(zhì)量,管理船檢證書副本。??在船舶及設(shè)備維護方面,給船舶以足夠的支持。??對船舶及設(shè)備維護情況進行定期檢查。相關(guān)的檢查可分解到海務(wù)、通導(dǎo)及電氣主管,如發(fā)現(xiàn)缺陷時,按P0901《不符合、缺陷報告和處理程序》規(guī)定執(zhí)行,找出原因,采取糾正措施,記錄并保存。??檢查船舶對于關(guān)鍵性設(shè)備和技術(shù)系統(tǒng)定期檢查和試驗的實施情況。??編制單船修理計劃報部門經(jīng)理。4.5 公司機務(wù)部

      ??制訂公司-月度維修保養(yǎng)管理規(guī)章制度。??根據(jù)各機務(wù)主管上報的修理計劃,審核并編制船舶修理計劃。5 實施步驟

      5.1 船舶-月度預(yù)防檢修計劃執(zhí)行

      5.1.1 機務(wù)主管根據(jù)船舶和設(shè)備情況,考慮適用的國際公約、船旗國和港口國強制性規(guī)則、船級社規(guī)范、制造廠商的要求和說明,及公司長期以來的經(jīng)驗積累,制訂出《船舶甲板部預(yù)防檢修計劃表》、《船舶輪機部預(yù)防檢修計劃表》,對預(yù)防檢修計劃表中的項目確定常規(guī)保養(yǎng)周期。

      5.1.2 船舶要根據(jù)船舶設(shè)備的實際情況及設(shè)備說明書規(guī)定的養(yǎng)護要求,編制《甲板部預(yù)防檢修計劃表》、《輪機部預(yù)防檢修計劃表》并作好月度預(yù)防檢修計劃;凡沒有預(yù)先編定的保養(yǎng)周期,可根據(jù)設(shè)備說明書或廠方提供的保養(yǎng)手冊要求船上自己確定養(yǎng)護周期,編制時要充分考慮接受船級社年檢和特檢項目的時間安排;執(zhí)行“循環(huán)檢驗”的船舶還必須將檢驗項目做合理的安排。此外凡有計劃廠修和塢修的也要對修理項目的維修做全盤考慮做好安排。船舶編制完成預(yù)防檢修計劃表于每年11月前將計劃上報公司機務(wù)部。

      5.1.3 大副、輪機長每月根據(jù)批準(zhǔn)的《甲板部預(yù)防檢修計劃表》、《輪機部預(yù)防檢修計劃表》,將當(dāng)月的維修保養(yǎng)項目,填入《月度預(yù)防檢修計劃和執(zhí)行記錄表》中。并將每月的維修保養(yǎng)項目分解到設(shè)備主管人員;要注意必須明確指出維修保養(yǎng)的內(nèi)容、要求及試驗驗收的規(guī)定。

      5.1.4 考慮到船舶運行的特殊情況,當(dāng)月不能按計劃完成的維修保養(yǎng)項目要盡量安排在下月完成,如果長期不能完成的,要報機務(wù)主管安排適當(dāng)時間完成,機務(wù)主管要做好備案。

      5.1.5 設(shè)備主管人員對設(shè)備進行養(yǎng)護后,要在《輪機日志》、《電氣日志》、《GMDSS電臺工作日志》、《檢修工作記錄簿》、《消防、救生設(shè)備登記檢查養(yǎng)護記錄簿》、《蓄電池維護或更換記錄》等上做好記錄,并且要報告部門長設(shè)備維護工作完成的簡要情況,部門長在《月度預(yù)防檢修計劃和執(zhí)行記錄表》上記錄。

      5.1.6 設(shè)備檢修后各種測量記錄要按規(guī)定填寫,由部門長簽字認(rèn)可。

      5.1.7 每月底大副、輪機長和電機員都必須將《月度預(yù)防檢修計劃和執(zhí)行記錄表》、《船舶應(yīng)急設(shè)備檢查執(zhí)行情況表》向船長報告,經(jīng)船長簽字后一式兩份,一份留存,一份報送機務(wù)主管。

      5.1.8 機務(wù)主管對船舶的執(zhí)行情況進行審閱。如發(fā)現(xiàn)問題,應(yīng)分析其原因,提供物質(zhì)支持或技術(shù)指導(dǎo)。

      5.1.9 船舶通信、導(dǎo)航設(shè)備的維護參照I 1001-07《船舶通信導(dǎo)航設(shè)備維護保養(yǎng)規(guī)定》,船舶通信、導(dǎo)航設(shè)備的檢查參照I 1002-02《關(guān)鍵性設(shè)備及技術(shù)系統(tǒng)定期檢查和測試規(guī)定》。6 船舶及設(shè)備缺陷情況的處理

      4.0設(shè)備設(shè)施維護保養(yǎng)程序

      1.0目的對公司設(shè)備設(shè)施進行維護保養(yǎng),為正常生產(chǎn)提供和維護所需要的設(shè)施,確

      保產(chǎn)品質(zhì)量符合要求。

      2.0范圍

      a)廠區(qū)內(nèi)環(huán)境設(shè)施保養(yǎng)

      b)廠房和場地維修保養(yǎng)

      c)工器具維修保養(yǎng)

      d)儀器設(shè)備設(shè)施維修保養(yǎng)

      3.0職責(zé)

      a)生產(chǎn)車間、品管部每年12月制定下的《設(shè)備設(shè)施維護計劃》,由

      總經(jīng)理審核批準(zhǔn)。

      b)生產(chǎn)車間負(fù)責(zé)對保證正常生產(chǎn)和確保產(chǎn)品質(zhì)量符合要求所需的設(shè)施進

      行跟進。

      c)品管部負(fù)責(zé)對生產(chǎn)工作環(huán)境進行控制。

      4.0程序

      4.1生產(chǎn)設(shè)施的定義:公司為保證正常生產(chǎn)和確保產(chǎn)品質(zhì)量符合要求所需的設(shè)施,包括:工作場所(車間、辦公場所等)、設(shè)備和工器具(包括生產(chǎn)設(shè)備、工具、器具、卡具、量具)、軟件(計算機網(wǎng)絡(luò))、公用設(shè)施(水、電、汽供應(yīng))、通訊設(shè)施、運輸設(shè)施等。

      4.2生產(chǎn)設(shè)施的提供

      4.2.1生產(chǎn)車間、品管部門根據(jù)使用的要求及公司發(fā)展的需要,說明設(shè)施名稱、型號

      規(guī)格、用途、技術(shù)參數(shù)、單價、數(shù)量等,經(jīng)總經(jīng)理審核,總經(jīng)理批準(zhǔn)后,由生產(chǎn)車間、品管部門負(fù)責(zé)組織安排采購或自制的有關(guān)事宜,采購部門具體實施采購;

      4.2.2需要自制的設(shè)施由使用部門提出,由生產(chǎn)品管部設(shè)計,經(jīng)總經(jīng)理審核,總經(jīng)理

      批準(zhǔn)后,由生產(chǎn)品管部門要求加工制造。

      4.3生產(chǎn)設(shè)施的驗收

      4.3.1采購或自制完成的設(shè)施,生產(chǎn)車間、品管部組織使用部門進行安裝調(diào)試,確認(rèn)

      滿足要求后,由生產(chǎn)品管部門和使用部門在驗收單上簽字驗收,并記錄設(shè)施名稱、型號規(guī)格、技術(shù)參數(shù)、單價、數(shù)量、隨機附件及資料等內(nèi)容。設(shè)施驗收單由生產(chǎn)品管部保管。低價值易耗品,如:工具、卡具、量具等由使用部門自行驗收;

      4.3.2驗收不合格的設(shè)施,生產(chǎn)車間、品管部與供貨商協(xié)商解決,并在設(shè)施驗收單上

      記錄處理結(jié)果;

      4.3.3生產(chǎn)車間、品管部對驗收合格的設(shè)施進行編號,建立《設(shè)備標(biāo)識卡》和設(shè)施檔

      案,并在《生產(chǎn)設(shè)備一覽表》上登記;

      4.3.4生產(chǎn)車間、品管部根據(jù)設(shè)施驗收單辦理入廠登記和建檔手續(xù);低值易耗品由倉

      管人員憑設(shè)施驗收單辦理入庫手續(xù)。

      4.4生產(chǎn)設(shè)施的使用、維護和保養(yǎng)

      4.4.1根據(jù)生產(chǎn)需要,生產(chǎn)車間、品管部組織編寫設(shè)施的操作規(guī)程,發(fā)放給使用部門。

      對于大型的精密設(shè)備或關(guān)鍵、特殊過程所用的設(shè)備必須有操作規(guī)程,相關(guān)的操作人員應(yīng)由品管部門負(fù)責(zé)培訓(xùn),考核合格后,持證上崗。

      4.4.2生產(chǎn)車間、品管部共同制定《設(shè)備維護保養(yǎng)計劃》,規(guī)定維護保養(yǎng)項目、頻次,交給使用部門執(zhí)行,各崗位負(fù)責(zé)人監(jiān)督檢查執(zhí)行情況。生產(chǎn)車間、品管部每季度收集《設(shè)備維修記錄表》,整理入檔并作為制定檢修計劃的依據(jù)。

      4.4.3生產(chǎn)車間、品管部每年12月制定下的《設(shè)備檢修計劃》,由總經(jīng)理審核批

      準(zhǔn)后發(fā)給各部門執(zhí)行。

      4.4.4日常生產(chǎn)中車間無法排除的故障,應(yīng)報生產(chǎn)品管部門檢修。檢修中的設(shè)施應(yīng)掛

      紅色檢修牌,檢修好的設(shè)施應(yīng)有使用部門負(fù)責(zé)人簽字驗收方可使用。生產(chǎn)品管部門應(yīng)將檢修發(fā)現(xiàn)的問題記錄在相應(yīng)的《設(shè)備標(biāo)識卡》上。

      4.4.5現(xiàn)場使用的設(shè)施應(yīng)有編號標(biāo)識,以便維護保養(yǎng)。

      4.5生產(chǎn)設(shè)施的報廢

      4.5.1對無法修復(fù)或無使用價值的生產(chǎn)設(shè)施,由生產(chǎn)品管部門填寫《設(shè)備報廢單》,經(jīng)

      總經(jīng)理批準(zhǔn)后作報廢處理,生產(chǎn)車間、品管部在《設(shè)備標(biāo)識卡》及《生產(chǎn)設(shè)施一覽表》中注明報廢情況。

      4.5.2對工具、卡具、量具等低值易耗品,由使用部門填寫《設(shè)施報廢單》,報生產(chǎn)車

      間、品管部領(lǐng)導(dǎo)批準(zhǔn)后,即可報廢。

      4.5.3等待報廢處理的設(shè)施應(yīng)掛報廢牌。

      4.6工作環(huán)境

      4.6.1財務(wù)部應(yīng)協(xié)助生產(chǎn)車間、品管部識別并管理為保證正常生產(chǎn)和確保產(chǎn)品質(zhì)量符

      合要求所需要的工作環(huán)境中人和物的因素,據(jù)生產(chǎn)作業(yè)需要,負(fù)責(zé)確定并提供作業(yè)場所必須的基礎(chǔ)設(shè)施,創(chuàng)造良好的工作環(huán)境,包括:配置與生產(chǎn)加工能力相適宜的車間廠房,并根據(jù)生產(chǎn)需要適當(dāng)裝修,以利于出口食品生產(chǎn)加工;配置必要的通風(fēng)、空氣調(diào)節(jié)和消防器材,保持適宜的溫度、濕度,以利于安全衛(wèi)生生產(chǎn); 生產(chǎn)車間、品管部對車間設(shè)施的配置和布局實行動態(tài)管理,要考慮人體工效學(xué)的要求,努力提高工作效率;

      確保員工生產(chǎn)符合勞動法規(guī)的要求。

      船舶過戶程序和手續(xù)

      先由舊船主辦理船舶營運證注銷手續(xù)。新船主持港航部門出具的《中華人民共和國船舶營業(yè)運輸證注銷登記證明書》到港航分局領(lǐng)取《船舶買賣、檢驗入(過)戶呈批表》,加具單位意見并蓋章(個體到村委或居委會加具意見并蓋章);持已蓋章的《船舶買賣、檢驗入(過)戶呈批表》到海事部門辦理船牌證件轉(zhuǎn)讓(過戶)手續(xù);填寫在港航分局領(lǐng)取的 《國內(nèi)水路運輸?shù)怯浭马椛暾埍怼贰ⅰ栋踩芾碡?zé)任書》,加具單位意見并蓋章(個體到村委或居委會加具意見并蓋章),個體還需由村委或居委會出具船舶買賣證明;持以上資料及新的船舶檢驗證書、船舶國籍證書、所有權(quán)登記證書等證件到港航分局進行初審,再由市港航局核發(fā)新的船舶營運證和水路運輸許可證

      6、(4)所入船籍港船舶監(jiān)督部門審批的船名核準(zhǔn)表;

      7、(5)原船籍港《船舶登記注銷證明》(復(fù)印件);

      8、(6)原船籍港船檢部門轉(zhuǎn)移的證書副本及圖紙檔案資料;

      9、(7)交通主管部門批準(zhǔn)文件(運輸船舶);

      10、(8)船舶正側(cè)彩照2張(3R)。

      11、辦理依據(jù):《廣東省船舶檢驗局船舶檢驗工作規(guī)定》 辦結(jié)時限:

      1、出戶:當(dāng)天

      2、入戶:檢驗完成后3個工作日內(nèi)發(fā)出船舶檢驗證書。收費標(biāo)準(zhǔn):中華人民共和國船舶檢驗局《船舶檢驗計費標(biāo)準(zhǔn)》(1998)及省船檢局有關(guān)規(guī)定。

      第五篇:船舶抵押程序

      船舶抵押程序

      一、船舶基本情況調(diào)查

      確定抵押船舶的具體情況,包括船名、船舶的識別號、船舶的登記號、船舶呼號、船籍港、船舶的種類、尺度、噸位等

      二、船舶所有權(quán)情況調(diào)查

      調(diào)查船舶所有人/抵押人的地址、聯(lián)系方式等,確認(rèn)船舶是否為其所有,取得船舶所有權(quán)登記證書,或船舶建造合同及所有權(quán)歸屬證明文書(在建船舶);調(diào)查船舶是否存在已抵押情況。

      三、船舶抵押確認(rèn)

      取得承租人對船舶設(shè)立抵押的確認(rèn)文件:船舶由多人共有的,取得2/3以上份額或約定份額的共有人同意設(shè)定抵押的證明文書,船舶已辦理光船租賃登記的,取得承租人同意船舶設(shè)定抵押的文書。

      四、船舶價值評估

      資產(chǎn)評估機構(gòu)進行船舶價值評估,出具船舶價值評估報告,雙方簽訂對船舶價值認(rèn)可的確認(rèn)書。

      五、簽訂抵押合同

      確定抵押合同中抵押擔(dān)保的債務(wù)的數(shù)額、抵押金額、抵押擔(dān)保債務(wù)的償還期限、抵押債務(wù)的利率。船舶原已設(shè)立抵押登記的,進行抵押權(quán)轉(zhuǎn)移的,取得抵押權(quán)轉(zhuǎn)移合同、抵押人通知抵押權(quán)人證明文書、原抵押權(quán)登記證。

      六、進行船舶抵押登記

      船舶抵押應(yīng)到船舶登記機關(guān)辦理船舶抵押登記,未經(jīng)登記的,不得對抗第三人。船舶抵押登記時應(yīng)提供抵押雙方的身份證,委托第三人辦理抵押登記的,還應(yīng)提供委托書及受委托人身份證。除此之外,應(yīng)填寫抵押權(quán)登記申請書。申請書的格式如附件所示。

      七、抵押權(quán)的實現(xiàn)

      船舶抵押權(quán)人實現(xiàn)船舶抵押權(quán)的方式有“折價”、“拍賣”和“變價”三種。在物權(quán)法第一百九十五條規(guī)定了抵押權(quán)實現(xiàn)的條件、方式和程序:債務(wù)人不履行到期債務(wù)或者發(fā)生當(dāng)事人約定的實現(xiàn)抵押權(quán)的情形,抵押權(quán)人可以與抵押人協(xié)議以抵押財產(chǎn)折價或者以拍賣、變賣該抵押財產(chǎn)所得的價款優(yōu)先受償。抵押權(quán)人與抵押人未就抵押權(quán)實現(xiàn)方式達(dá)成協(xié)議的,抵押權(quán)人可以請求人民法院拍賣、變賣抵押財產(chǎn)。雖有上述規(guī)定,但實踐中鮮有抵押權(quán)人與抵押人就抵押權(quán)的實現(xiàn)方式達(dá)成協(xié)議。因缺乏配套的程序性規(guī)定,抵押權(quán)人也未能直接向法院申請拍賣、變賣財產(chǎn),抵押權(quán)仍通過訴訟予以實現(xiàn)。審判實踐中,抵押權(quán)的實現(xiàn)通常是經(jīng)過訴訟程序,取得生效判決以作為執(zhí)行根據(jù),再拍賣抵押船舶實現(xiàn)抵押權(quán)。

      在具體實現(xiàn)程序上,可參照廈門海事法院裁定中信銀行廈門分行申請實現(xiàn)船舶抵押權(quán)案——根據(jù)民事訴訟法實現(xiàn)擔(dān)保物權(quán)的規(guī)定適用非訟程序,船籍港及船舶所在地海事法院對此享有管轄權(quán),并有權(quán)作出可以作為執(zhí)行名義的終審裁定。

      4、委托書及受委托人身份證原件及復(fù)印件(適用于委托他人辦理)

      5、承租人同意船舶設(shè)定抵押的文書(適用于已辦理光船租賃登記的船舶

      6、資產(chǎn)評估機構(gòu)出具的船舶價值評估報告及復(fù)印件(適用時)
