      申報高、中級專業(yè)技術(shù)職務(wù)資格

      評審材料有關(guān)問題的說明

      為了使評審材料報送、填寫真實規(guī)范,確保職稱評審工作順利進行,現(xiàn)就我市申報高、中級專業(yè)技術(shù)職務(wù)資格,報送評審材料過程中,有關(guān)具體問題說明如下:

      一、申報人須提供的評審材料類別、數(shù)量

      1.《山東省專業(yè)技術(shù)職稱評審表》1份(A4紙型,原件,雙面打印,左側(cè)裝訂)。

      2.《專業(yè)技術(shù)職稱評審簡表》一式3份(A4紙型,原件,雙面打印)。

      3.《專業(yè)技術(shù)人員申報評審職稱情況一覽表》一式3份(A3紙型,原件),須使用《山東省職稱評審管理系統(tǒng)》打印生成。

      4.《濟南市推薦評審高、中級專業(yè)技術(shù)職務(wù)任職資格量化賦分表》1份(A4紙型,原件)。

      5.反映本人任現(xiàn)職以來專業(yè)技術(shù)水平、能力、業(yè)績的業(yè)務(wù)工作總結(jié)1份;代表性著作、論文、作品,成果及獎勵證書等(原件)。其中,《專業(yè)技術(shù)人員申報評審職稱情況一覽表》中“任現(xiàn)職以來取得的成果及受獎情況”(以下簡稱 1

      “成果及受獎”)欄填報數(shù)量不超過5件;“任現(xiàn)職以來發(fā)表、出版的論文、著作、作品等”(以下簡稱“論文、著作、作品”)欄填報數(shù)量不超過5件,其中代表性著作不超過2部。填寫《專業(yè)技術(shù)人員申報評審職稱情況一覽表》時,“成果及受獎”欄,應(yīng)按要求填寫“核心提綱、解決問題、效果”等內(nèi)容;“論文、著作、作品”欄,應(yīng)按要求填寫“刊號、提綱、觀點、文章字數(shù)”等內(nèi)容。

      6.學(xué)歷及學(xué)位證書(原件。如學(xué)歷證書丟失,須提交畢業(yè)生登記表原件)。

      7.現(xiàn)專業(yè)技術(shù)職務(wù)資格證書、單位聘書(原件)。

      8.任現(xiàn)職期間各考核登記表(原件)。

      9.有效期內(nèi)的外語考試合格證或成績單(原件),符合免試條件的,須提供經(jīng)市人事局審核同意的《山東省海外留學(xué)回國等部分專業(yè)技術(shù)人員申報專業(yè)技術(shù)資格免于外語和計算機應(yīng)用能力考試審核表》1份(A4紙型,原件)。

      10.全國專業(yè)技術(shù)人員計算機應(yīng)用能力考試合格證(原件),符合免試條件的,須提供經(jīng)市人事局審核同意的《計算機應(yīng)用水平考試免試審批表》或《山東省海外留學(xué)回國等部分專業(yè)技術(shù)人員申報專業(yè)技術(shù)資格免于外語和計算機應(yīng)用能力考試審核表》1份(A4紙型,原件)。

      11.《推薦申報專業(yè)技術(shù)職稱“六公開”監(jiān)督卡》1份(A4紙型,原件)。

      12.改系列申報專業(yè)技術(shù)職務(wù)任職資格(指同級別的),須呈報《改系列申報專業(yè)技術(shù)職稱評審表》一式4份(A4紙型,原件),原《專業(yè)技術(shù)職稱評審表》或《專業(yè)技術(shù)職務(wù)呈報表》1份(原件),并報送反映其工作變動后業(yè)務(wù)水平、業(yè)績情況等證明材料。

      13.破格申報的,須報送經(jīng)市人事局審核同意的《破格參加評審審批表》1份(A4紙型,原件)。

      二、申報人所在單位、主管部門、呈報部門應(yīng)履行的手續(xù)及要求

      1.所在單位、主管部門、呈報部門要對申報材料認真審查,嚴格把關(guān),經(jīng)公示無異議的,申報人、審核人、單位負責人分別在申報材料的相應(yīng)意見欄中簽字,加蓋印鑒后,連同其他申報材料,由呈報部門按規(guī)定時間報送到指定地點。

      2.符合破格申報專業(yè)技術(shù)職務(wù)任職資格的,須由單位和有關(guān)主管部門出具破格推薦報告1份,并由呈報部門核準簽署意見,加蓋印鑒。

      3.呈報部門對申報材料審核后,同時上報通過《山東省職稱評審管理系統(tǒng)》生成的數(shù)據(jù)匯總盤1份。

      4.呈報部門填報《專業(yè)技術(shù)職務(wù)任職資格申報人員花名冊》2份(A4紙型,原件,加蓋印鑒,附數(shù)據(jù)盤)。

      三、其他要求

      1.呈報的申報材料必須手續(xù)完備,內(nèi)容齊全,字跡清晰,不得涂改、漏頁、缺頁。

      2.申報材料須按要求裝入規(guī)范的材料袋內(nèi),實際材料件數(shù)與材料目錄必須相符。

      3.各級人事部門要嚴格按照規(guī)范的程序,認真組織好申報專業(yè)技術(shù)職務(wù)任職資格的評議推薦,申報材料的填報、審查、審核、呈報工作,確保提供的評審材料真實、準確、有效。報送省里評審的,按省各評委會辦事機構(gòu)有關(guān)要求辦理。
