(一)教師資格認(rèn)定申請(qǐng)表

1.申請(qǐng)人員網(wǎng)上填報(bào)完畢后用A4紙自行正反面打印(原件一式兩份，將思想品德鑒定表與申請(qǐng)表一起用膠水將左側(cè)粘貼整齊)。

2.思想品德鑒定表(一式兩份)按要求填好并蓋好公章。（戶(hù)口不在北京的人員申請(qǐng)認(rèn)定教師資格，其思想品德鑒定，既要有工作單位的公章,還要有本市存放人事檔案機(jī)構(gòu)（人才服務(wù)中心）的公章，沒(méi)有工作單位的本市申請(qǐng)人，其鑒定表上本人所在居委會(huì)蓋章后加蓋存檔機(jī)構(gòu)的公章，有工作單位的本市申請(qǐng)人，單位有組織人事部門(mén)公章的鑒定表上蓋單位組織人事部門(mén)公章，單位無(wú)組織人事部門(mén)公章的鑒定表上蓋所在單位公章并加蓋檔案存放部門(mén)（人才服務(wù)中心）公章）。

（9）根據(jù)本次教師資格認(rèn)定工作要求，申請(qǐng)者需提供電子版、紙質(zhì)照片，電子版尺寸應(yīng)在20K之內(nèi)，像素為114（寬）×156（高），照片擴(kuò)展名只能為.jpg，系統(tǒng)不接受其它格式的文件；紙質(zhì)照片同電子版尺寸，背面注明姓名、單位。

5、資格認(rèn)定

市、縣（區(qū)）教師資格認(rèn)定機(jī)構(gòu)根據(jù)職責(zé)權(quán)限，按照《江蘇省教師資格專(zhuān)家審查委員會(huì)組織辦法（試行）》的規(guī)定，組織教師資格專(zhuān)家審查委員會(huì)對(duì)申請(qǐng)人的材料進(jìn)行綜合審查，并提出審查意見(jiàn)。

5月5日前，市、縣（區(qū)）教師資格認(rèn)定機(jī)構(gòu)按照《關(guān)于做好取得教師資格人員信息報(bào)送工作的通知》（蘇教師[2003]6號(hào))要求，將代碼表、資格認(rèn)定人員名冊(cè)、工作總結(jié)書(shū)面報(bào)市教育局政治處。

6、證書(shū)發(fā)放

2009年6月底前，對(duì)經(jīng)審查符合教師資格認(rèn)定條件者，頒發(fā)教師資格證書(shū)。

第五篇：對(duì)外漢語(yǔ)教師資格認(rèn)證鄭重聲明

鄭重聲明

近來(lái)發(fā)現(xiàn)某些機(jī)構(gòu)打著“國(guó)際漢語(yǔ)教師協(xié)會(huì)”的旗號(hào)在招生，說(shuō)他們的漢語(yǔ)教師證書(shū)是得到認(rèn)可的。經(jīng)慎重調(diào)查，我們鄭重聲明如下：

一、經(jīng)核實(shí)，“國(guó)際漢語(yǔ)教師協(xié)會(huì)”為子虛烏有。根據(jù)國(guó)家法律規(guī)定，任何行業(yè)協(xié)會(huì)的成立均須經(jīng)過(guò)協(xié)會(huì)所在區(qū)域的國(guó)家民政局、工商局、公安局、稅務(wù)局的聯(lián)合批復(fù)方能成立。經(jīng)查“國(guó)際漢語(yǔ)教師協(xié)會(huì)”均未得到以上行政部門(mén)注冊(cè)備案，故該組織為子虛烏有。

二、經(jīng)調(diào)查，打著“國(guó)際漢語(yǔ)教師協(xié)會(huì)”名義，發(fā)漢語(yǔ)教師資格證書(shū)的機(jī)構(gòu)有好幾家，但這幾家發(fā)的證書(shū)均不一樣，所認(rèn)證的過(guò)程、考試的內(nèi)容、考試的時(shí)間均不一樣。進(jìn)步應(yīng)正這個(gè)協(xié)會(huì)的子虛烏有，其發(fā)證行為純屬為個(gè)人行為，沒(méi)有任何法律效應(yīng)，等同賣(mài)假證，屬于國(guó)家嚴(yán)厲打擊之范疇。

三、對(duì)外漢語(yǔ)教師的培訓(xùn)屬于特殊教育行業(yè)，必須經(jīng)過(guò)國(guó)家教育行政機(jī)關(guān)的特別審批方能展開(kāi)培訓(xùn)，只有取得相對(duì)應(yīng)的辦學(xué)許可證方能開(kāi)展相應(yīng)培訓(xùn)。公司性質(zhì)的機(jī)構(gòu)不得開(kāi)展相應(yīng)的教師培訓(xùn)，但以上打著“國(guó)際漢語(yǔ)教師協(xié)會(huì)”招生的機(jī)構(gòu)均為公司性質(zhì)，不具備任何辦學(xué)資質(zhì)。

四、“IPA國(guó)際注冊(cè)漢語(yǔ)教師資格證”發(fā)證單位為“IPA國(guó)際認(rèn)證協(xié)會(huì)”，是經(jīng)過(guò)國(guó)家行政部門(mén)審批設(shè)立的，在國(guó)家工商總局、民政部門(mén)、公安局均有注冊(cè)備案，其備案號(hào)為：***。

五、目前只有“IPA國(guó)際注冊(cè)漢語(yǔ)教師資格證”是得到國(guó)家人事部唯一授權(quán)認(rèn)可并可在全國(guó)進(jìn)行考試的；也只有IPA國(guó)際注冊(cè)漢語(yǔ)教師資格證書(shū)才能在國(guó)家人事部的官方網(wǎng)站上查詢(xún)及注冊(cè)備案。

六、目前也只有IPA國(guó)際注冊(cè)漢語(yǔ)教師資格證書(shū)在全國(guó)實(shí)行了統(tǒng)一考試，唯有該考試才有國(guó)家人事部審定的指定考試大綱、考試指南、考試用書(shū)，并且在全國(guó)各個(gè)主要城市均有考點(diǎn)。其他某些機(jī)構(gòu)稱(chēng)謂的漢語(yǔ)教師資格考試，既無(wú)相關(guān)國(guó)家權(quán)力部門(mén)給予的授權(quán)，也無(wú)相應(yīng)考試大綱、考試指南及考試用書(shū)，更無(wú)遍及全國(guó)的考點(diǎn)。既不合法更無(wú)權(quán)威可言。

七、“京師環(huán)宇”以北京師范大學(xué)為依托，是經(jīng)過(guò)國(guó)家教委特殊審批的具有對(duì)外漢語(yǔ)教師培訓(xùn)資質(zhì)的正規(guī)辦學(xué)機(jī)構(gòu)，其辦學(xué)許可證號(hào)為：京教***。上課均在與當(dāng)?shù)睾献鞯膰?guó)家重點(diǎn)院校內(nèi)。如北京師范大學(xué)、中國(guó)傳媒大學(xué)、華中師范大學(xué)、云南大學(xué)、沈陽(yáng)師范大學(xué)、黑龍江大學(xué)、合肥師范學(xué)院、太原師范學(xué)院等。
