“比較洛伐他汀與血脂康”的演講稿

尊敬的各位評委、各位老師，親愛的同學(xué)們：

大家晚上好！

我是來自14藥學(xué)1班的xxx，非常榮幸能夠站在這里代表我們14藥學(xué)1班參加本次ppt匯報大賽，同時也非常榮幸能夠得到許軍老師與楊尊榮老師的指導(dǎo)，非常感謝他們的悉心栽培。

首先，我想問大家一個問題，大家知道全球死亡率最高的是哪一類型的疾病嗎？——沒錯，就是心腦血管疾病。那么今天我們要給大家?guī)淼木褪侵委熜哪X血管疾病藥品中的調(diào)血脂藥——比較洛伐他汀與血脂康。

下面我將從三個反面來比較洛伐他汀與血脂康，其中包括洛伐他汀和血脂康的發(fā)現(xiàn)歷程、化學(xué)結(jié)構(gòu)、物理性質(zhì)、藥理作用、以及他們的臨床應(yīng)用與毒副反應(yīng)的區(qū)別。

讓我們來看一下洛伐他汀的發(fā)現(xiàn)歷程：20世紀70年代初，日本北里醫(yī)藥研究所的Endo等幾位微生物科學(xué)家從一種名為桔青酶的霉菌代謝物中意外發(fā)現(xiàn)了一種未知物，該未知物能抑制羥甲戊二酰輔酶A還原酶的活性，從而能明顯地降低血漿中的膽固醇。日本科學(xué)家將新發(fā)現(xiàn)的這種可能降低血脂的未知物命名為康帕定，也就是我們熟悉的美伐他汀，從此便開創(chuàng)了羥甲戊二酰輔酶A還原酶抑制劑類調(diào)血脂藥的新紀元。

隨后，德國的默克公司的研究人員在紅曲霉菌和土曲霉菌培養(yǎng)液中發(fā)現(xiàn)洛伐他汀，將其開發(fā)成片劑，并在1987年首次在美國上市，成為第一個上市的他汀類藥物。

大家通過這張ppt可以發(fā)現(xiàn)，洛伐他汀是由三個部分組成的，包括母核六氫萘環(huán)、必須活性基團四氫吡喃內(nèi)酯環(huán)以及2-甲基丁酸脂基。由于它含有一個內(nèi)脂，一個外脂，我們可以推斷出這種物質(zhì)不穩(wěn)定，而且在酸性和堿性條件容易水解。同時由于內(nèi)脂環(huán)羥基的存在，它還容易發(fā)生氧化反應(yīng)，生成二酮吡喃。

下面讓我們來看一下它的物理性質(zhì)：洛伐他汀是一種白色結(jié)晶粉末，不溶于水，易容于三氯甲烷、DMF、丙酮、乙腈，略溶于甲醇、乙醇、丙異醇、丁醇等。由于六元內(nèi)酯環(huán)的存在，它不穩(wěn)定、易氧化、水解，因此在儲存過程中要格外小心。同時，其六元內(nèi)脂環(huán)的開環(huán)產(chǎn)物與甲基戊二酰輔酶A部分相似，因此可以競爭性抑制羥甲基戊二酰輔酶A還原酶。又因為羥甲基戊二酰輔酶A還原酶是內(nèi)源性膽固醇合成中的限速酶，因此他可以抑制膽固醇的合成，從而降低血漿中膽固醇的含量，達到降血脂的作用。

不知道大家有沒有發(fā)現(xiàn)，洛伐他汀本身其實是沒有活性的，它需要在體內(nèi)經(jīng)過代謝，將其內(nèi)酯環(huán)水解成開鏈的β-羥基酸衍生物才具有抑酶活性，這是因為該開鏈的羥基酸結(jié)構(gòu)部分恰好與甲基戊二酰輔酶A還原酶的底物甲基戊二酰輔酶A的戊二酰部分具有相似性，由于酶識別錯誤，與其結(jié)合后失去催化活性，使膽固醇合成受阻，故能有效的降低血漿中的膽固醇。因此洛伐他汀是一種前藥，其本身無活性，在體內(nèi)代謝開環(huán)，最后經(jīng)膽汁排出。

接下來我們看看血脂康的發(fā)展歷程：90年代初，北京大學(xué)的張茂良教授等人培育出一種能穩(wěn)定產(chǎn)生他汀類物質(zhì)的紅曲，命名為特制紅曲，這就是血脂康主要成分。其中紅曲最早發(fā)現(xiàn)于中國，已有一千多年的生產(chǎn)、應(yīng)用歷史，是中國及周邊國家特有的大米發(fā)酵傳統(tǒng)產(chǎn)品。紅曲古代稱丹曲，既是中藥，又是食品，是用紅曲霉屬真菌接種于大米上經(jīng)發(fā)酵制備而成的。作為傳統(tǒng)中藥，明代李時珍在《本草綱目》評價它說：“此乃人窺造化之巧者也”，“奇藥也”，在許多古代中藥典集中嶺載該品具有活血化瘀、健脾消食等功效，用于治療食積飽脹、產(chǎn)后惡露不凈、痕滯腹痛和跌打損傷等癥。

血脂康這個藥是由綠葉制藥公司1995年在我國上市，并于2001年獲得國家發(fā)明專利證書。血脂康的主要成分為紅曲，而紅曲中含有百分之六洛伐他汀，因此血脂康中有效成分便是洛伐他汀。除此之外還含有其他天然復(fù)合他汀：包括羥基酸鈉、內(nèi)酯脫水物、內(nèi)酯及脫水物等，大多數(shù)成分均有降脂活性；黃酮類物質(zhì)：作用廣泛，可抑制CYP3A4及P-gp活性，增加天然他汀生物利用度；麥角甾醇：具有干擾膽固醇吸收促溶抗炎等作用；多種氨基酸：具有有廣泛的生理保護作用調(diào)節(jié)免疫功能、抗氧化。因此，這些物質(zhì)能和洛伐他汀協(xié)同作用，副作用更小，安全性更高。同時，陳蘭英等教授等的研究結(jié)果顯示,血脂康抑制羥甲基戊二酸單酰輔酶還原酶的活性優(yōu)于辛伐他汀和洛伐他汀。

接下來讓我們一起了解一下洛伐他汀與血脂康的臨床應(yīng)用。洛伐他汀是用于治療高膽固醇血癥和混合型高脂血癥。而血脂康具有除濕祛痰，活血化瘀，健脾消食的功效。用于脾虛痰瘀阻滯癥的氣短、乏力、頭暈、頭痛、胸悶、腹脹、食少納呆等；高血脂癥；也可用于由高血脂癥及動脈粥樣硬化引起的心腦血管疾病的輔助治療。由此可見血脂康比洛伐他汀適用人群更廣。

最后我們一起來看一下洛伐他汀和血脂康的毒副作用比較。洛伐他汀最常見的不良反應(yīng)為胃腸道不適、腹瀉、脹氣，其它還有頭痛、皮疹、頭暈、視覺模糊和味覺障礙。偶爾可引起血氨基轉(zhuǎn)移酶可逆性升高。因此需監(jiān)測肝功能。少見的不良反應(yīng)有陽萎、失眠。罕見的不良反應(yīng)有肌炎、肌痛、橫紋肌溶解，表現(xiàn)為肌肉疼痛、乏力、發(fā)燒，并伴有血肌酸磷酸激酶升高、肌紅蛋白尿等，橫紋肌溶解可導(dǎo)致腎功能衰竭，但較罕見。并且與免疫抑制劑、葉酸衍生物、煙酸、吉非羅齊、紅霉素等合用可增加肌病發(fā)生的危險。

因為血脂康的主要成分是洛伐他汀，所以血脂康也具有腸胃道不適的不良反應(yīng)，如胃痛、腹脹、胃部灼熱等；以及引起血清氨基轉(zhuǎn)移酶和肌酸磷酸激酶可逆性升高。所以出現(xiàn)血清氨基轉(zhuǎn)移酶升高時可以使用護肝藥護理。由于血脂康是一種天然藥物，作用溫和，因此一般耐受性良好，大部分副作用輕微而短暫。罕見乏力、口干、頭暈、頭痛、肌痛、皮疹、膽囊疼痛、浮腫、結(jié)膜充血和泌尿道刺激癥狀。

尊敬的各位評委、老師，親愛的同學(xué)們：

大家好！我是大化坪中心學(xué)校六年級的學(xué)生，我的名字是程鈺菲！今天我演講的題目是《弘揚民族精神展示學(xué)子風采》。

一個人如果沒有一種精神，那他永遠也不會長大；一個企業(yè)如果沒有一種精神，那他就不會在競爭中求得生存；一個政黨，一個民族，一個國家，如果沒有一種精神，那他永遠都不會發(fā)展壯大。

先說兩件大家都知道的事：2012年倫敦奧運會，中國游泳小將葉詩文輕松打破世界紀錄并獲奧運金牌，西方輿論發(fā)出的“嘖嘖”聲不那么自然，大量評論介于驚奇和懷疑之間。甚至有記者說她是“只會訓(xùn)練比賽拿獎牌的機器人”。

大家也應(yīng)該知道：82年前的9月18，日本關(guān)東軍炸毀柳條湖南滿鐵路，悍然發(fā)動侵華戰(zhàn)爭；81年后的9月18，兩名日本人闖入我釣魚島，把所謂的“購島”鬧劇推向了高潮。

說到這里相信同學(xué)們和我一樣憤怒。在憤怒的同時我也在思考：我的祖國擁有五千年悠久的歷史，我的祖先創(chuàng)造了無數(shù)燦爛的文化，中國人民站起來了，香港，澳門順利回歸了，人民生活水平提高了，中國加入世貿(mào)組織了,成功舉辦奧運會了，航天事業(yè)飛速發(fā)展了，中國日益強大了，可為什么仍有人對中國虎視眈眈、尋釁滋事？在思考中我找到了答案：那就是我們的國家今天并不是世界上最強大的！在中國開始與西方競爭但仍處于劣勢時，類似不愉快遭遇有可能還會發(fā)生。

中華要崛起，中華要復(fù)興，中國要富強就必須在提升國家“硬實力”的同時，還要提升國家“軟實力”，也就是提升全民族的“精、氣、神！”

而我們青少年就擔負著這神圣而艱巨的任務(wù)，我們就要接過前一代人的接力棒，勇于開拓，積極進取，大力弘揚中華民族的偉大精神，展示出南高莘莘學(xué)子的風采！

那么身處校園的我們究竟該如何做呢？我想首先我們應(yīng)該做一個大氣的人。大氣就是弘揚誠勇、追求卓越；大氣就是仁愛、博學(xué)，是真、是善、是美；大氣就是面對成功時的淡然，面對失敗時的坦然；大氣就是有朋自遠方來不亦樂乎，是知之為知之、不知為不知，是君子坦蕩蕩，言必信、行必果，是大道之行，天下為公！大氣的最高境界，便是正氣，凜然的正氣！那就是：面對邪惡不低頭，保護柔弱不自負；敢向不平揮劍，肯如老牛俯首！

其次，我們應(yīng)該具有一顆剛毅的心。它就是“一劍磨十年”的恒心；就是丘吉爾“決不、決不、決不能放棄”的魂魄；就是做事的專一，“弱水三千，我只取一瓢飲”；就是一生只做一件事。袁隆平一生只做一件事，成了中國雜交水稻之父；華佗一生只做一件事，成了中華傳奇千古的神醫(yī)；愛因斯坦一生只做一件事，成了偉大的科學(xué)家。就連偉大的毛主席，一生也只做一件事，成立一個新中國。比爾?蓋茨曾說：“其實，我沒有什么過人之處，只不過是我提前知道了一生只做一件事的道理，而我把這件事做得比別人好而已。” 一生只做一件事，想不成功都難！那么讓我們從現(xiàn)在開始也只做一件事，那就是——為中華民族的偉大復(fù)興而拼命讀書！我堅信：走在復(fù)興的道路上，只要有知識的陪伴，我們就有勇氣與天公試比高，只要有民族精神的存在，我們就能勇敢彎弓射大雕；只要有大家的努力，我們就能無畏而言：數(shù)風流人物，還看今朝！

“我們是五月的花海，用青春擁抱時代，我們是初生的太陽，用生命點燃未來………..”每當聽到這激揚人心的旋律時，我總會感受到我們當代青年所肩負的重大使命，那就是愛我中華，復(fù)興中華。曾記得有這樣一個故事：一枚一分錢的硬幣不知何故躺在馬路中央，人流穿梭，它被踩踏致滿身污垢。直到一名小男孩看到后將他撿起。周圍的人告訴他:“孩子，那只是一分錢，什么也買不了，它多臟啊，把它扔了吧?！钡?，那個男孩卻用手帕將它擦凈，大聲的告訴大家：“這上面印有國徽，做為一個中國人應(yīng)該熱愛自己的祖國?！倍嗝粗赡鄣哪橗?，多么樸實的語言，但他卻賦予了愛國最真切的內(nèi)涵。那么，什么是愛國呢?

愛國是一種信仰，是一種力量，是一個源泉。打開歷史的長卷。因為愛國，岳飛背刻“精忠報國”；因為愛國，吉鴻昌“恨不抗日死”；也因為愛國，李克強總理親自前往四川雅安指揮抗震救災(zāi)。再翻開新中國的創(chuàng)業(yè)史，我們又看見了新中國的英雄：有錢三強、鄧稼先、楊振寧等學(xué)識淵博的科學(xué)家；有焦裕祿、任長霞、楊利偉等兢兢業(yè)業(yè)的軍人干部；更有王進喜、徐虎、李素麗等艱苦奮斗的普通勞動者。他們在各行各業(yè)的戰(zhàn)線上，釋放著自己的光和熱，這就是愛國。

我們不能忘記，昔日的圓明園現(xiàn)在已為一片廢墟。我們不能忘記，擁有5000年文明的大國淪為歐美列強的樂土。我們不能忘記，小日

本對我國釣魚島的侵犯。我們更不能忘記祖國統(tǒng)一大業(yè)尚未完成。今天，祖國的發(fā)展已進入到一個新的時期，新的時期，賦予了我們新的使命，新的時代，賦予了愛國新的內(nèi)涵。那么，做為新世紀的小學(xué)生，我們應(yīng)該怎樣去愛國呢?

Discover Yourself Ever since the accelerated development of human civilization, perpetual scientific discoveries of nature had accompanied mankind all along the journey.It was by discovering the world around us that human beings have been able to lead a more sophisticated life.However, as G.K.Chesterton once said, “One may understand the cosmos, but never the ego;the self is more distant than any star.” Similarly, our life is a journey of unrelenting efforts to discover more of ourselves.When I was 6 I constantly pictured myself as an elegant and skilled pianist rendering Chopin’s nocturne beautifully on the stage, receiving warm applause after my performance.As I furthered my piano lessons, it was hard to be oblivious of my non-gifted skills.Sometimes I couldn’t help but have this feeling of despair when I woke up to the reality and discovered that my goal was unrealistic.Brutal but real, it was the first time I had discovered that I couldn’t have it all.When I was 12 I thought the coolest job in the entire universe was to be a surgeon.I spent hours and hours watching the drama “Grey’s Anatomy”, and couldn’t help falling in love with the kind of life those doctors lead on the screen.I was utterly mesmerized by the scalpel and all those fancy medical terms they use.However, I stopped dreaming about being a surgeon when I discovered that it was the idea of being a surgeon that appealed to me, not what it truly was.Therefore, the dream ended and I discovered some more about myself.This year I turned 16, I could proudly say I do not dream to be but ascertain to pursue being a lawyer in the future.Since junior high school I was always awarded the “most eloquent debater” in every single debate I participated in.I cannot only think logically but also deliver my thoughts promptly.My huge interest in philosophy also adds to my reasoning whenever I tried to argue on something equivocal.Being aware of the obstacles and hardships on the way to becoming a great lawyer never panics me, for the simple reason that I have this faith and passion in what I am aspiring after.G.K.Chesterton was absolutely correct, the most complicated part of comprehending the world is the study of the somehow smaller world—ourselves.I subsequently realized that growing up is the process of discovering ourselves;we get to know what we truly are, and become who we dream to be.As we shape ourselves, and as we shape our lives, we gradually become more aware of our limits and potentials and consequently learn to adjust to what’s out there.And by that, ladies and gentleman, I’m proud to say that I’ve discovered part of myself, still discovering today standing on this stage, and hopefully a lot more in the future.I will always keep this embedded into my heart.That things do not change, we do.
