醫(yī)療工作制度
一、醫(yī)生必須以救死扶傷、防病治病、為公民的健康服務(wù)為宗旨。
二、醫(yī)生上崗要著裝整潔;使用文明用語;關(guān)心體貼病人;態(tài)度和藹有禮貌;耐心地解答問 題;有計劃地安排病人就診。
三、對病人要認真檢查,做好登記,實行首診負責(zé)制。
四、醫(yī)生應(yīng)嚴格按照診療常規(guī)操作,門診病歷必須認真填寫,首診病歷要求有主訴、重要病 史、既往史、各種陽性體征、必要的陰性體征、診斷或印象診斷、治療及處理意見、醫(yī) 生簽字。復(fù)診病歷要求有主訴、體檢、診斷、處理、簽名。
五、醫(yī)生要采用保證療效、經(jīng)濟便宜的治療方法,科學(xué)用藥、合理用藥,盡量減輕病人負擔(dān)。

消毒培訓(xùn)制度
一、門診部醫(yī)務(wù)人員人人重視消毒滅菌工作,做到操作中避免感染源傳播。
二、定期參加區(qū)市消毒培訓(xùn),本門診部一月一次總結(jié)消毒工作。
三、新來人員必須先行消毒知識學(xué)習(xí),經(jīng)考試合格后方可上崗。
四、門診設(shè)有消毒監(jiān)督檢查專職人員,隨時檢查監(jiān)督,消毒工作的每一步操作環(huán)節(jié),發(fā)現(xiàn)問題隨時匯報,及時糾正。
五、定期請衛(wèi)生監(jiān)督部門前來檢查指導(dǎo)工作。
疫情報告管理制度
1、執(zhí)行職務(wù)的醫(yī)療保健人員、衛(wèi)生防疫人員、個體開業(yè)醫(yī)生為責(zé)任疫情報告人。責(zé)任疫情報告人發(fā)現(xiàn)法定傳染病病人、疑似病人、病原攜帶者應(yīng)在規(guī)定的時限內(nèi),向區(qū)衛(wèi)生防疫站報告,必要時做訂正死亡報告。
2、各醫(yī)療衛(wèi)生機構(gòu)及人員要認真學(xué)習(xí)《傳染病防治法》、《突發(fā)公共衛(wèi)生事件應(yīng)急處理條例》等法律、法規(guī)和傳染病防治知識,熟練掌握傳染病診斷、報告、隔離消毒及疫情處理的程序,切實增強傳染疫情報告意識,發(fā)現(xiàn)傳染病例要認真做好傳染病登記,填寫傳染病報告卡,在規(guī)定時限內(nèi)向衛(wèi)生防疫機構(gòu)報告。
3、在各醫(yī)療機構(gòu)防保科設(shè)立傳染病疫情報告員。傳染病報告員接到臨床醫(yī)生的傳染病報告要進行核實診斷,認真做好記錄,并及時上報當(dāng)?shù)匦l(wèi)生防疫機構(gòu)。認真開展疫情主動監(jiān)測工作,對臨床有關(guān)科室的門診日志和住院登記至少每旬開展一次疫情搜查,發(fā)現(xiàn)漏報的傳染病病例,要及時進行補報。
4、責(zé)任疫情報告人發(fā)現(xiàn)甲類傳染病、傳染性非典型肺炎和乙類傳染病中的艾滋病、肺炭疽的病人、脊髓灰質(zhì)炎的病人、病原攜帶者或疑似病人,城鎮(zhèn)于2小時內(nèi),農(nóng)村于6小時內(nèi)通過傳染病疫情監(jiān)測信息系統(tǒng)進行報告。對其他乙類傳染病人,疑似病人和傷寒、副傷寒、痢疾、梅毒、淋病、乙型肝炎、白喉、瘧疾的病原攜帶者,城鎮(zhèn)于6小時內(nèi),農(nóng)村于12小時內(nèi)通過傳染病疫情監(jiān)測信息進行報告。對丙類傳染病和其它傳染病,應(yīng)當(dāng)在24小時內(nèi)通過傳染疫情監(jiān)測信息系統(tǒng)進行報告。
5、傳染病暴發(fā)、流行時,責(zé)任疫情報告人應(yīng)當(dāng)以最快的通訊方式向當(dāng)?shù)匦l(wèi)生防疫機構(gòu)報告疫情。接到疫情報告的衛(wèi)生防疫機構(gòu)應(yīng)當(dāng)以最快的通訊方式報告上級衛(wèi)生防疫機構(gòu)和當(dāng)?shù)卣l(wèi)生行政部門,衛(wèi)生行政部門接到報告后,應(yīng)當(dāng)立即報告當(dāng)?shù)卣?/p>
6、流動人員中的傳染病人,病原攜帶者和疑似傳染病人的傳染病報告、處理由診治地負責(zé),其疫情登記、統(tǒng)計由戶口所在地負責(zé)。
7、區(qū)衛(wèi)生防疫機構(gòu)應(yīng)當(dāng)對轄區(qū)內(nèi)種類醫(yī)療保健機構(gòu)的疫情登記報告和管理情況定期進行核實、檢查、指導(dǎo)。
8、任何單位和個人對突發(fā)事件和傳染病疫情,不得隱瞞、緩報、謊報或者授意他人隱瞞、緩報、謊報,否則將依法追究責(zé)任。
9、任何單位和個人發(fā)現(xiàn)傳染性非典型肺炎病人或者疑似傳染性非典型肺炎病人時,必須2小時內(nèi)以最快的方式向區(qū)衛(wèi)生防疫站報告。區(qū)衛(wèi)生防疫站發(fā)現(xiàn)疫情或者接到疫情報告,應(yīng)當(dāng)立即報告上級防疫機構(gòu)和當(dāng)?shù)匦l(wèi)生行政部門。
10、區(qū)衛(wèi)生防疫站疫情管理人員應(yīng)定期編發(fā)疫情簡報,對重大疫情進行預(yù)測、預(yù)報。
傳染病病例處置工作制度
制訂傳染病預(yù)防控制工作方案和重大疫情應(yīng)急處置技術(shù)方案,組建傳染病疫情應(yīng)急處置隊伍,做好傳染病診斷試劑、生物制品、防治藥品、消毒除害藥械等的應(yīng)急儲備工作。認真落實傳染病疫情的預(yù)測預(yù)警措施和專報制度,加強信息傳遞和溝通,及時發(fā)現(xiàn)傳染病疫情苗頭,一旦在重大活動參加人員中出現(xiàn)傳染病疫情,必須迅速啟動應(yīng)急預(yù)案,采取果斷措施,嚴防疫情擴散與蔓延。
(一)預(yù)防為主
堅持“預(yù)防為主”的衛(wèi)生工作方針,按照“早發(fā)現(xiàn)、早診斷、早隔離、早治療”的傳染病防治原則,提高警惕,加強監(jiān)測,及時發(fā)現(xiàn)病例,采取有效的預(yù)防與治療措施,切斷傳播途徑,迅速控制重大疫病在我市的傳播和蔓延。
(二)依法防治
為有效切斷傳染病的傳播,根據(jù)有關(guān)法律、法規(guī)、結(jié)合重大疫病的流行特征,在采取預(yù)防控制措施時,對留院觀察病例、疑似病例、臨床診斷病例及實驗室確認病例依法隔離治療,對疑似病例、臨床診斷病例及實驗室確診病例的密切接觸者依法實行隔離和醫(yī)學(xué)觀察。
(三)開展醫(yī)療救治
按照醫(yī)療救治方案啟動指定的專門醫(yī)療救治網(wǎng)絡(luò),開展病人接診、收治和轉(zhuǎn)運工作,實行重癥和普通病人分別管理,對疑似患者及時排除或確診。做好消毒隔離、個人防護和醫(yī)療垃圾處理工作,防止院內(nèi)交叉感染和污染。并參與組成專家組負責(zé)全市重大疫病的會診工作。
預(yù)防和控制重大疫情要堅持“早、小、嚴、實”的方針,對留院觀察病例、疑似病例、臨床診斷病例及實驗室確診病例,要做到“及時發(fā)現(xiàn)、及時報告、及時治療、及時控制”。同時,對疑似病例、臨床診斷病例及實驗室確診病例的密切接觸者要及時采取隔離控制措施,做到統(tǒng)一、有序、快速、高效。
醫(yī)療廢物管理制度
一、醫(yī)療廢棄物必須與生活垃圾分類管理。
二、按《醫(yī)療廢物管理條例》規(guī)定要求建立醫(yī)療廢棄物專用的密封貯存室,標(biāo)明標(biāo)志并加鎖,防止醫(yī)療廢棄物的流失,泄漏和擴散。
三、由門診指定專人對輸液管和注射器類(經(jīng)過消毒、毀形)、玻璃器皿類、組織液樣本、試管或其它類進行分類,分別用易燃無毒的軟包裝袋密封裝好,并貼上分類標(biāo)識卡。
四、分類包裝好的醫(yī)療廢棄物由清潔工送至存入醫(yī)療廢棄物的貯存室的專用密封桶中。
五、定期由后勤人員將醫(yī)療廢物送至縣衛(wèi)生局指定地點,針頭等類物品定期焚燒處理。并如
實填寫收運交接時間、種類、數(shù)量的醫(yī)療廢棄物轉(zhuǎn)移聯(lián)單和經(jīng)辦人簽名。
六、醫(yī)療廢棄物貯存室每天由清潔工清洗,并用2500mg/L的含氯消毒劑噴灑消毒。
七、對有關(guān)接觸處理醫(yī)療廢棄物的人員定期進行知識培訓(xùn)。

要領(lǐng):兩腳跟靠攏并齊,兩腳尖向外分開約60度;兩腿挺直;小腹微收,自然挺胸;上體正直,微向前傾;兩肩要平,稍向后張;兩臂自然下垂,手指并攏自然微屈,拇指尖貼于食指的第二節(jié),中指貼于褲縫;頭要正,頸要直,口要閉,下頜微收,兩眼向前平視 口令:跨立。
要領(lǐng):左腳向左跨出約一腳之長,兩腿自然伸直,上體保持立正姿勢,身體重心落于兩腳之間。而手后背,左手握右手腕,右手手指并擾自然彎屈,手心向后??诹睿荷韵?。
要領(lǐng):左腳順腳尖方向伸出約全腳的三分之二,兩腿自然伸直,上體保持立正姿勢身體重心大部分落于右腳??诹睿合蛴遥ㄗ螅D(zhuǎn)
要領(lǐng):以右(左)腳跟為軸,右(左)腳跟和左(右)腳掌前部同時用力,使身體和腳一致向右(左)轉(zhuǎn)90度,體重落在右(左)腳,左(右)腳取捷徑迅速靠攏右(左)腳,成立正姿勢。轉(zhuǎn)動和靠腳時,兩腿挺直,上身保持立正姿勢。口令:向后--轉(zhuǎn)。
要領(lǐng):按向右轉(zhuǎn)的要領(lǐng)向后轉(zhuǎn)180度??诹睿糊R步--走。
要領(lǐng):左腳向正前方邁出約75厘米著地,身體重心前移,右腳照此法動作;上體正直,微向前傾;手指輕輕握攏,拇指貼于食指第二節(jié);兩臂前后自然擺動,向前擺臂時,肘部彎屈,小臂自然向里合,手心向內(nèi)稍向下,拇指根部對正衣扣線,并與最下方衣扣同高 口令:立--定。
要領(lǐng):齊步和正步時,聽到口令,左腳再向前大半步著地,兩腿挺直,右腳取捷徑迅速靠攏左腳,成立正姿勢??诹睿憾紫隆?/p>
要領(lǐng):右腳后退半步,臀部坐在右腳跟上(膝蓋不著地),兩手自然放在兩膝上,上體保持正直。蹲下過久,可自行換腳??诹睿浩鹆?。
