(一)教師資格認(rèn)定申請(qǐng)表
1.申請(qǐng)人員網(wǎng)上填報(bào)完畢后用A4紙自行正反面打印(原件一式兩份,將思想品德鑒定表與申請(qǐng)表一起用膠水將左側(cè)粘貼整齊)。
2.思想品德鑒定表(一式兩份)按要求填好并蓋好公章。(戶口不在北京的人員申請(qǐng)認(rèn)定教師資格,其思想品德鑒定,既要有工作單位的公章,還要有本市存放人事檔案機(jī)構(gòu)(人才服務(wù)中心)的公章,沒(méi)有工作單位的本市申請(qǐng)人,其鑒定表上本人所在居委會(huì)蓋章后加蓋存檔機(jī)構(gòu)的公章,有工作單位的本市申請(qǐng)人,單位有組織人事部門(mén)公章的鑒定表上蓋單位組織人事部門(mén)公章,單位無(wú)組織人事部門(mén)公章的鑒定表上蓋所在單位公章并加蓋檔案存放部門(mén)(人才服務(wù)中心)公章)。
(二)教育學(xué)、教育心理學(xué)成績(jī)(要求和條件)。
(三)學(xué)歷證書(shū)
1.相應(yīng)的學(xué)歷證書(shū)原件和復(fù)印件(各類(lèi)申請(qǐng)資格學(xué)歷要求)
2.應(yīng)屆畢業(yè)生申請(qǐng)認(rèn)定教師資格,需提供學(xué)校出具的應(yīng)屆生畢業(yè)證明。(僅適用于春季認(rèn)定)
3.軍隊(duì)院校學(xué)歷要有全國(guó)高等學(xué)校學(xué)生信息咨詢與就業(yè)指導(dǎo)中心認(rèn)證處出具的認(rèn)定證明,地址:北京市海淀區(qū)北四環(huán)中路238號(hào)柏彥大廈506室,聯(lián)系電話:010-61139123。
4.國(guó)外學(xué)歷需提供教育部留學(xué)生服務(wù)中心的學(xué)歷鑒定認(rèn)證書(shū)。
5.有國(guó)家計(jì)劃招生的普通高等學(xué)校(民辦)畢業(yè)生,需提交由學(xué)校招生部門(mén)和就業(yè)指導(dǎo)部門(mén)出具統(tǒng)招統(tǒng)分證明。
(四)體檢證明原件和原件的封面、個(gè)人信息頁(yè)及結(jié)論頁(yè)的復(fù)印件(查看體檢標(biāo)準(zhǔn)和指定醫(yī)院)
(五)普通話證書(shū)原件和復(fù)印件(查看普通話水平要求)
(六)身份證原件和復(fù)印件
(七)本人近期免冠正面小二寸證件照片一張(與上傳照片為同一底版)

2012年陜西省中小學(xué)教師資格教育基礎(chǔ)理論知識(shí)考試成績(jī)已經(jīng)公布,現(xiàn)就考試成績(jī)合格人員填報(bào)體檢表有關(guān)事宜通知如下:
一、成績(jī)查詢
2012年陜西省中小學(xué)教師資格教育基礎(chǔ)理論知識(shí)考試閱卷、登分工作由省教育廳組織實(shí)施,5月28日成績(jī)已向社會(huì)公布,請(qǐng)申請(qǐng)人員盡快登陸陜西省教育廳門(mén)戶網(wǎng)站(004km.cn/),按提示查詢本人考試成績(jī)并打印《陜西省2012年中小學(xué)教師資格教育基礎(chǔ)理論知識(shí)考試成績(jī)報(bào)告單》。
二、填報(bào)體檢表人員
申請(qǐng)高級(jí)中學(xué)、中等職業(yè)學(xué)校、初級(jí)中學(xué)、小學(xué)、幼兒園教師資格人員中:
1、報(bào)名時(shí)已審核認(rèn)定教育基礎(chǔ)理論知識(shí)四科全部免試的人員;
2、參加了教育基礎(chǔ)理論知識(shí)考試,每科考試成績(jī)均在60分及以上的人員;
3、參加了2012年陜西省中小學(xué)教師資格教育基礎(chǔ)理論知識(shí)考試的應(yīng)屆普通高校非師范教育類(lèi)畢業(yè)生,每科考試成績(jī)均在60分及以上,且為陜西生源的考生。
三、辦理時(shí)間
6月4日(星期一)—8日(星期五)
四、辦理地點(diǎn)
西安市教育考試中心南院(文藝南路194號(hào))原報(bào)名點(diǎn),聯(lián)系電話:87274558。
五、攜帶資料
1、教育基礎(chǔ)理論知識(shí)四門(mén)課程免試人員攜帶身份證、準(zhǔn)考證、近期2寸免冠彩色照片4張。
2、參加了教育基礎(chǔ)理論知識(shí)考試的考生攜帶本人身份證、準(zhǔn)考證、《陜西省2012年中小學(xué)教師資格教育基礎(chǔ)理論知識(shí)考試成績(jī)報(bào)告單》、近期2寸免冠彩色照片4張。
六、工作程序
教育基礎(chǔ)理論知識(shí)考試合格人員到相應(yīng)工作窗口核對(duì)成績(jī),領(lǐng)取《申請(qǐng)認(rèn)定教師資格人員體檢表》《申請(qǐng)認(rèn)定教師資格人員申請(qǐng)表》,現(xiàn)場(chǎng)填寫(xiě)體檢表及申請(qǐng)表上相關(guān)內(nèi)容,粘貼照片后交市教師資格認(rèn)定辦公室。
七、注意事項(xiàng)
教育基礎(chǔ)理論知識(shí)考試合格人員體檢的具體時(shí)間、地點(diǎn)和要求請(qǐng)關(guān)注西安教育網(wǎng)、西安市招生考試網(wǎng)。

1.同一申請(qǐng)人不能在一年內(nèi)同時(shí)或先后申請(qǐng)兩種以上(含兩種)教師資格。
2.各級(jí)各類(lèi)學(xué)校師范教育類(lèi)專(zhuān)業(yè)應(yīng)屆畢業(yè)生(不含成人教育畢業(yè)生),具備教師資格認(rèn)定條件,在畢業(yè)前最后一學(xué)期,由學(xué)校有關(guān)部門(mén)按照教師資格認(rèn)定機(jī)構(gòu)規(guī)定時(shí)間統(tǒng)一辦理。
3.各級(jí)各類(lèi)學(xué)校非師范教育類(lèi)專(zhuān)業(yè)應(yīng)屆畢業(yè)生,可在最后一個(gè)學(xué)期持學(xué)校出具的應(yīng)屆畢業(yè)生證明,按社會(huì)認(rèn)定要求由個(gè)人在規(guī)定時(shí)間向教師資格認(rèn)定機(jī)構(gòu)申請(qǐng)。通過(guò)認(rèn)定后,憑身份證、領(lǐng)證通知單和畢業(yè)證書(shū)原件、復(fù)印件領(lǐng)取教師資格證書(shū)。
4.軍隊(duì)院校學(xué)歷要有認(rèn)定學(xué)歷的專(zhuān)門(mén)機(jī)構(gòu)出具的認(rèn)定證明。
認(rèn)定學(xué)歷的專(zhuān)門(mén)機(jī)構(gòu)是:全國(guó)高等學(xué)校學(xué)生信息咨詢與就業(yè)指導(dǎo)中心
未達(dá)到國(guó)家法定退休年齡的中國(guó)公民,具備《教師法》規(guī)定的條件,遵守?fù)?jù)相關(guān)法律,熱愛(ài)教育事業(yè),具有良好的思想品德,具備教師法規(guī)定的學(xué)歷,教育教學(xué)能力經(jīng)測(cè)試合格者,可以取得教師資格。根據(jù)《教師資格條例》第五章第十三條規(guī)定,申請(qǐng)人員均可向其單位或戶籍所在地的教師資格認(rèn)定機(jī)構(gòu)申請(qǐng)。對(duì)于戶口不在北京的人員申請(qǐng)認(rèn)定教師資格,其思想品德鑒定,既要有工作單位的證明,又要有在本市存放人事檔案的機(jī)構(gòu)(人才服務(wù)中心)出具的相關(guān)證明;戶籍和人事檔案均不在北京的,應(yīng)回戶籍所在地申請(qǐng)認(rèn)定教師資格。

4、受理材料

(7)思想品德鑒定表
(8)體檢表(由教師資格認(rèn)定機(jī)構(gòu)統(tǒng)一投放材料袋)
(9)根據(jù)本次教師資格認(rèn)定工作要求,申請(qǐng)者需提供電子版、紙質(zhì)照片,電子版尺寸應(yīng)在20K之內(nèi),像素為114(寬)×156(高),照片擴(kuò)展名只能為.jpg,系統(tǒng)不接受其它格式的文件;紙質(zhì)照片同電子版尺寸,背面注明姓名、單位。
5、資格認(rèn)定
市、縣(區(qū))教師資格認(rèn)定機(jī)構(gòu)根據(jù)職責(zé)權(quán)限,按照《江蘇省教師資格專(zhuān)家審查委員會(huì)組織辦法(試行)》的規(guī)定,組織教師資格專(zhuān)家審查委員會(huì)對(duì)申請(qǐng)人的材料進(jìn)行綜合審查,并提出審查意見(jiàn)。
5月5日前,市、縣(區(qū))教師資格認(rèn)定機(jī)構(gòu)按照《關(guān)于做好取得教師資格人員信息報(bào)送工作的通知》(蘇教師[2003]6號(hào))要求,將代碼表、資格認(rèn)定人員名冊(cè)、工作總結(jié)書(shū)面報(bào)市教育局政治處。
6、證書(shū)發(fā)放
2009年6月底前,對(duì)經(jīng)審查符合教師資格認(rèn)定條件者,頒發(fā)教師資格證書(shū)。
